Job Summary: Nurse practitioners (NP) provide health care services to patients who may have acute episodic conditions or chronic problems, as well as those seeking health evaluation, preventive care and/or education and counseling. NPs provide care in areas within KP in primary care or specialty departments consistent with their educational preparation following the Regional Standardized Procedures and Process Protocols for Ambulatory and Hospital-Based Practice. They perform complete physical examinations, including ordering, interpreting and evaluating diagnostic tests and examinations. Assume on-going responsibility for each patient from discovery of the disorder to recovery. Diagnose and treat both chronic and episodic disorders including complications of otherwise normal processes. Establish and document the health care plan and diagnosis, maintain records of each case that are sufficiently complete for any physician reviewing them to evaluate the effectiveness of the plan. Consistently demonstrates the knowledge, skills, abilities and behaviors necessary to provide superior and culturally sensitive service to each other, to our members and to customers, contracted providers and vendors. Essential Responsibilities: Collect subjective data (obtain history). Collect objective data (perform a physical exam). Develop an assessment (diagnosis). Develop and implement a management plan. Facilitate the referral of patients to the appropriate health facilities, agencies and resources within the community. Order pharmacologic interventions. Develop and implement an educational plan. Plan for appropriate follow-up care. Consistently supports compliance and the Principles of Responsibility (Kaiser Permanentes Code of Conduct) by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licenser requirements (if applicable), and Kaiser Permanentes policies and procedures.

On an Average Day You Will: (includes, but not limited to) Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record Manage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care services Complete the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners Assure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols Collaborate with the interdisciplinary group to create, review and revise the patient's plan of care Fulfill the obligation of requested and/or accepted assignments Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues Complete all other duties as assigned Requirements: Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program Must have current nursing licensure in State Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience Must have accreditation from an approved certifying body for advanced practice nursing as required by the State Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics) Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order Join a Team that embraces the reverence of life!
