Position Summary Vitae Health is seeking a dedicated weekend Nurse Practitioner to provide high-quality, patient-centered medical care to residents in a skilled nursing facility every weekend. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life.
