The Nurse Practitioner, if providing palliative care, cares for patients facing chronic, complex, and/or life-threatening conditions at various locations within the flow of patient care. S/he provides assessments, disease and treatment education, and symptom management to meet the needs of palliative care patients and their families. The Nurse Practitioner helps the patient and family maintain their maximum level of comfort and coping. S/he provides care and support in collaboration with the PCP and specialists; and ensures compliance with practice standards and codes. S/he flexes schedule to meet agency visit needs and may be asked to provide after-hours emergency visits and/ or admissions. Position Specific Responsibilities If providing Hospice Care: Performs routine and emergency assessments: Completes routine and emergency recertification visits on each patient entering their 3rd benefit period or beyond (including admissions and transfers). Completes routine and emergency assessments on each patient as indicated by departmental policy. Records observations, treatments, and other pertinent information. Communicates with Medical Director, Attending Physician, DCS and IDT as directed regarding findings from the visits and any measures to alleviate symptoms. Provides physical and emotional care to support the patient and family in period of crisis. Prevents unwarranted hospitalizations by meeting patient and family needs in the home. In our programs that serve palliative care, the additional following outpatient responsibilities are needed: Performs routine and emergency assessments related to outpatient palliative care visits. Performs bedside palliative care patient/family evaluation in the patient’s place of residence. Assists with diagnosis, treatment, and management of acute and chronic health conditions. Orders, performs, and interprets laboratory and radiology tests within scope of professional practice. Prescribes medications including controlled substances to the extent delegated and licensed. Orders treatments and durable medical equipment as indicated. Performs other therapeutic or corrective measures as indicated, including urgent care. Consults with palliative care physician or designees as needed, informs primary physician of services provided and collaborates with other physicians as needed. Assists in all facets of care coordination for palliative care referrals. Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided. Complies with applicable laws and regulations with respect to Collaborative Agreements. Initiates reimbursement for services rendered. Assists patients and families with goal setting related to treatment options and advanced care planning. Provides training and continuing education for staff. Assists in development of clinical practice guidelines/standards in support of quality palliative care. If providing Hospice Face to Face Visits: Performs routine and emergency assessments: Completes routine and emergency recertification visits on each patient entering their 3rd benefit period or beyond (including admissions and transfers). Education and/or Experience Master’s degree in Nursing required. If providing Hospice Care: Minimum of one (1) year nursing practice or equivalent experience required; hospice, oncology, or home health and/or long-term care preferred. If providing Palliative Care: Minimum of five (5) years nursing experience required; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred. The Central Intake Clinician responds to inquiry call and/or emails/faxes. Conducts initial intake assessments, provides community referrals when appropriate, and coordinate admissions processes. Handles all aspects of managing the patient flow from the initial inquiry, assessment, documentation, internal and external communication and coordinates admission or referral to the appropriate level of care based on the patient’s clinical presentation. Delivers patient services in a respectful manner which promotes dignity and patient satisfaction. Identifies safety needs including ability to detect possible signs of abuse and neglect and report same to Supervisor on duty. Contacts emergency services on behalf of patients in crisis when necessary for wellness check, while keeping the patient engaged on the phone until emergency services have arrived, and patient is safe. Identifies special mobility and equipment needs of patients. Identifies need for interpreter services, when necessary, prior to admission and notifies accepting facilities of need. Assess and interprets age and disability needs to assess level of care needed and place patient at nearest appropriate facility that can accommodate the patient's needs. Communicates with providers in a professional manner to include all pertinent information regarding the patient's presentation for provider to determine acceptance or denial based on criteria provided. Communicates with patients and families in a manner sensitive to their age, disability and cultural needs and involves families of minors or significant others in decision making related to treatment as appropriate. Nurse Practitioner, Acute Cardiac Transfer (Weekends Only)

Job Description The Nurse Practitioner examines and treats patients independently and in autonomous collaboration with other health care professionals following established standards and practices. Ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. Relies on education, experience, professional training and judgement to accomplish responsibilities. A wide degree of creativity and latitude is expected. Works under minimal supervision under the direction of the collaborating physician, as appropriate. May lead and direct the work of others within scope of practice. Responsibilities TECHNICAL TASKS Perform bedside palliative care patient/family evaluation. Assist with diagnosis, treatment, and symptom management of acute and chronic health conditions. Order, perform, and interpret treatments within scope of professional practice. Consult program medical director as needed; comply with applicable laws and regulations with respect to Collaborative agreements. Initiate reimbursement for services rendered Assist in development of clinical practice guidelines/standards/protocols in support of quality palliative care. Provide training and continuing education for staff on palliative care. Meet monthly to provide reports and outcomes in relation to Palliative Care team goals and objectives. The leadership role of the Practitioner includes patient and family expectations; group dynamics to affect positive change; and provides a strong image as a role model. The practitioner will also supervise all RNs who are assisting with the program. INTERPERSONAL RELATIONS Communicates and collaborates with physicians, nurses and other multidisciplinary team members involved in the care of patients. Acts as patient advocate constantly monitoring, tracking, assessing and evaluating quality of care Promotes communication with referring physicians and the entire team regarding plan of care Promotes professional development of the team. Qualifications Program development experience Current license to practice profession nursing in Louisiana Master's degree in Nursing and/or Nurse Practitioner license Current license to practice as APN in Louisiana. Current BLS Certficiation
