Essential Duties & Responsibilities: Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs. Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions. Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence. Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills. Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems. Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care. PPG Informatics Specialist

*Inpatient and/or Ambulatory clinical experience preferred, frequent travel within Allen County and surrounding areas* Summary The PPG Informatics Specialist will be responsible to work with providers, clinical and other staff to optimize usage of EHR through education and implementation of optimal build and/or workflow improvements. Provides education and support in accordance with organization goals, acts as liaison on behalf of provider/clinical EHR needs and implements ongoing tactics for knowledge sustainment. Understands all workflows impacting providers and has a basic understanding of build requirements for the EHR and associated 3rd party applications. Responsible for project planning and collaborates with providers, co-workers, leaders, I/S and other pertinent departments throughout the health system. Proactively identifies opportunities for improvement in application functionality or workflow processes related to electronic health record. Education Minimum requirement of a Bachelor's degree in nursing, informatics, healthcare or related field; or the equivalent of 8 years of experience in a clinical, healthcare, informatics related setting. Licensure/Certification Must obtain credentials as Epic Credentialed Trainer in relevant EHR modules. Experience Minimum requirement of 4 years experience in clinical/healthcare setting, direct clinical ambulatory/practice experience preferred, EHR experience preferred. Experience with Lean preferred. Other Qualifications Demonstrates clinical understanding in physician practices as well as industry trends/regulations.
