Position Summary Vitae Health is seeking a dedicated weekend Nurse Practitioner to provide high-quality, patient-centered medical care to residents in a skilled nursing facility every weekend. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life. Clinical (Both Ambulatory and Inpatient) A. Assesses the physical and psychosocial status of clients by means of interview, health history, physical examination, and diagnostic studies. B. Recognizes deviations from normal in the physical assessment. Works in collaboration with a physician in formulating treatment plans for health problems and follow-up. C.
