Company : Allegheny Health Network Job Description : GENERAL OVERVIEW: Functions in an expanded role as a professional nurse practitioner at West Penn and Jefferson Regional Hospitals, performs acts of medical diagnosis or prescription of medical therapeutic or corrective measures in collaboration with and under the direction of a physician licensed to practice medicine. (20%) Attend and provide coverage for high risk deliveries and stabilization of neonates, provide transport coverage for sick neonates as warranted per attending physician and appropriately initiates emergency evaluation and treatment. (20%) Coordinate plan of care within area of specialty, and coordinate discharge/transfers, including dictation of discharge and transfer summaries. (10%) Provide support, educate, and communicate with patient, family, interdisciplinary team, and community health services as appropriate. Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice. 15% Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physician(s).10% Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds and provides wound care10% Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommends community resources as appropriate.10% Triages patient telephone calls, provides monitoring and continuity of care10% Participates, develops, and implements performance and/or quality improvement initiatives.
