Job Summary: We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Responsibilities: Build strong relationships with the facility and maintain a patient-centered approach Collaborate with the healthcare team to provide comprehensive care to patients Conduct on-site visits and remote work from home, supported by a dedicated scribe team Develop and implement individualized care plans for patients Perform chronic disease management and monitoring Provide education and support to patients and families Participate in quality improvement initiatives to reduce hospitalizations Requirements: Active NP or PA license in the state of practice 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills Masters' degree in Nursing or related field Benefits: Hybrid Role!
