As a member of the clinical team, the Nurse Practitioner will bring an advanced knowledge and diverse skill-set to assess the healthcare of chronically ill geriatric members, in collaboration with physicians and interdisciplinary teams. Essential to this role is the desire and ability to aid and empower members in maximizing their quality of life, ensuring they can live as independently as possible in their homes and their communities. Responsibilities This hands-on role includes a host of responsibilities for these diverse members, including: Performing health histories and in-depth medical exams for new members, to determine preliminary Plans of Care Initiating, evaluating and reviewing medical orders, and identifying opportunities to improve care, when possible Carrying out various procedures and documenting all care and recommendations Ordering, interpreting and following up on lab tests and results Requesting consultations from medical and non-medical in-house and external providers, including physicians, therapists, dietitians, social workers and other specialists Educating staff, members, caregivers and families on essential care functions to ensure members have maximum independence at home and in centers Completing ongoing assessments to determine Plan of Care changes or improvements based on member feedback and medical evaluations Qualifications All applicants must be certified registered nurse practitioners, with a valid Pennsylvania nursing license. Additionally, candidates must be willing to participate in continuing education/workshops to maintain his/her valid license. Required Qualifications: For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care For PAs: Current NCCPA certification and state licensure Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations Active and unrestricted driver's license In addition to a driver's license, access to reliable transportation Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice) Ability to gain a collaborative practice agreement, if applicable in your state Ability to navigate varied environments and to position oneself as needed to perform job duties Preferred Qualifications: 1+ years of clinical experience as an APC (long-term care setting preferred) Experience in geriatric medicine, long-term care, senior living or home care setting Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs Understanding of Geriatrics and Chronic Illness Understanding of Advanced Illness and end of life discussions Proficiency with electronic medical records and technology Compensation for this specialty generally ranges from $53 - $79 per hour. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. On an Average Day You Will: (includes, but not limited to) Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record Manage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care services Complete the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners Assure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols Collaborate with the interdisciplinary group to create, review and revise the patient's plan of care Fulfill the obligation of requested and/or accepted assignments Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues Complete all other duties as assigned Requirements: Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program Must have current nursing licensure in State Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience Must have accreditation from an approved certifying body for advanced practice nursing as required by the State Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics) Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order Join a Team that embraces the reverence of life!
