As a Behavioral Health APC (PMHNP), you will play a critical role within an interdisciplinary care team, bringing psychiatric expertise into a home-based and population health model of care. Primary Responsibilities: Conduct telehealth-based psychiatric evaluations for patients with serious mental illness and substance use conditions Diagnose, treat, and prescribe evidence-based pharmacologic and non-pharmacologic interventions Provide bridge stabilization through medication management and follow-up care for high-risk patients Work with interdisciplinary care team to ensure connection to community mental health services. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
