Summary GENERAL SUMMARY: Responsible for meeting patient’s priority needs by assessing the patient, participating in interdisciplinary care planning, providing and/or delegating care executing therapeutic physician-and nurse-initiated interventions, and evaluating the patient's response to care and progress toward clinical outcomes. Contributes to an efficient and effective work environment through activities including professional communication and documentation and/or resourcing of others. Responsible for the appropriate delegation of nursing care to licensed and unlicensed personnel according to Nebraska statute. PRINCIPAL JOB FUNCTIONS: 1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values. 2. *Collects subjective and objective data and contributes to the assessment of the patient and documents appropriately in the patient chart. 3. *Evaluates and communicates patient’s progress to ordering provider. 4. *Facilitates patient’s transition to next level of care if needed. 5. *Utilizes appropriate resources to plan and provide nursing services that are safe, effective and financially responsible. 6. *Collaborates with the patient, family, providers and others in the provision of nursing care. 7. *Assists in identifying teaching, learning and discharge needs and communicates with providers, patient and family. 8. *Demonstrates leadership in the professional practice setting and the profession. 9. *Makes accurate and timely observation and documents changes in the patient’s condition in the patient chart accurately and according to policy. 10. *Performs a huddle with provider regarding identified questions, individualized patient care and wellness needs. 11. *Initiates assessment based on the patients presenting problems or needs and the physician’s medical diagnosis. 12. Determines nursing diagnosis and priorities for care with special consideration given to patient teaching, goal setting, psychosocial well-being and functional status. 13. *Assesses tolerance of activity levels and adjusts program as determined by patient’s ability. 14. *Addresses and applies interpretation of diagnostic/monitoring data to the plan of care. 15. Appropriately interacts with patients/families from a variety of developmental and sociocultural backgrounds; seeks interpreter assistance as appropriate. 16. Provides information to patients and families, which reduces anxiety and conveys an attitude of acceptance, sensitivity and caring. 17. *Initiates communication with health care team members regarding treatment planning; informs appropriate health care team members (including physicians) of patient’s condition/rehab status. 18. Completes clinical/non-clinical assignments based on departmental workload and the qualifications and competencies of self and other staff members. 19. Assesses individual educational needs, barriers for learning and learning styles and provides appropriate learning experiences; reviews, develops and provides educational materials appropriate to patient needs. 20. *Provides education to patient and significant others regarding disease entities, risk factor management and behavior modification and promotes primary, secondary and tertiary prevention. 21. * Performs specialty clinic nursing skill set (Blood transfusions, Chemotherapy administration, Wound care, IV therapy, injections, cardiology stress tests and aids with nuclear medicine testing. 22. *Schedules referrals department work queue and assist with Recall List. 23. Works efficiently and closely with other departments for the care and treatment of the patient. 24. *Administers medication according to policy and provider orders. 25. Assists the provider with examinations and procedures. 26. *Performs cardiolyte and stress tests and coordinates scheduling of these tests 27. *Chart preparation is thorough, complete, and done prior to the start of each office day. 28. Maintains a neat, organized, and safe environment for the patients, visitors, and MMC team. 29. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise. 30. Performs other related projects and duties as assigned. Essential Job functions are marked with an asterisk “*. EDUCATION AND EXPERIENCE: Graduation from an accredited professional nursing program required. BSN preferred. Current Registered Nurse License in the State of Nebraska required or approved compact state of residence as defined by the Nebraska Nurse Practice Act. Must provide evidence of certification and/or instruction for practice area as required by State or Federal standards/regulations. Chemotherapy Certification required prior to administering chemotherapy independently. OTHER CREDENTIALS / CERTIFICATIONS: Current Basic Life Support (BLS) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network. Advanced Cardiac Life Support (ACLS) required within three (3) months of hire. Chemotherapy Competency Education required prior to administering chemotherapy independently.

Prioritizes data collection based on the patient’s immediate condition or the anticipated needs of the patient. Synthesizes and analyzes available data, information and knowledge relevant to the situation to identify patterns and variances and determine appropriate nursing diagnosis. Develops a plan of care that prescribes the strategies and alternatives to attain expected outcomes. Implements identified plan of care, documenting relevant data. Evaluates patient’s response to and effectiveness of interventions and evaluates patient’s progress toward established outcomes. Facilitates patient’s transition to next level of care. Utilizes appropriate resources to plan and provide nursing services that are safe, effective and financially responsible. Collaborates with the patient, family, providers and others in the provision of nursing care. Provides education for the patient and family according to their needs, abilities, preferences and readiness to learn. Demonstrates leadership in the professional practice setting and the profession. Attends monthly staff meetings as well as complete all mandatory training. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise. Participates in meetings, committees and lean projects as assigned. Performs other related projects and duties as assigned. EDUCATION AND EXPERIENCE: Graduation from an accredited professional nursing program required. BSN preferred. Current Registered Nurse License in the State of Nebraska required or approved compact state of residence as defined by the Nebraska Nurse Practice Act. Must provide evidence of certification and/or instruction for practice area as required by State or Federal standards/regulations. OTHER CREDENTIALS / CERTIFICATIONS: Basic Life Support (CPR) certification required.
