Summary GENERAL SUMMARY: Responsible for meeting patient priority needs by assessing, participating in interdisciplinary care planning, providing and /or delegating care and evaluating patient responses and progress toward clinical outcomes. Contributes to an efficient and effective work environment through activities including professional communication and documentation, precepting and/or resourcing of others. Supervises Licensed Practical Nurses and/or other support staff in the delivery of patient care as described in Nebraska Statute 38. PRINCIPAL JOB FUNCTIONS: 1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values. 2. *Initiates assessment and reassessment based on the patient’s presenting problems/needs in accordance with Bryan Medical Center’s procedures. 3. *Synthesizes pertinent initial and ongoing patient data to identify nursing priorities for care and desired outcomes/goals. 4. *Integrates appropriate standards of care into the patient’s plan of care based on the diagnosis and patient-specific needs. 5. *Executes therapeutic and/or supportive interventions. 6. *Provides education for the patient and family to meet identified patient needs according to their needs, abilities, preferences and readiness to learn. 7. *Evaluates the patient’s response to and effectiveness of interventions. 8. *Evaluates patient’s progress toward established outcomes. 9. *Incorporates cultural considerations into the plan/delivery of care. 10. *Includes family, significant other, or identified support person in the care of the patient as appropriate. 11. *Documents patient care and related information according to Bryan Medical Center procedures for documentation. 12. *Provides care in collaboration with other members of the interdisciplinary team. 13. *Facilitates patient’s transition to the next level of care. 14. Assumes responsibility in a shared or delegated assignment and supports team member’s practices through timely communication. 15. Serves as a role model to colleagues, students and other care providers. 16. *Actively participates in continued learning opportunities and continued self-development pertinent to the work unit and duties of the job. 17. Integrates evidence based research findings into practice in accordance with policy and procedure. 18. Engages in department specific and organizational performance improvement initiatives. 19. Appropriately utilizes all safety equipment when indicated and as defined by organizational procedure (i.e. lifting equipment, PPE, sharps, etc.). 20. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise. 21. Participates in meetings, committees and department projects as assigned. 22. Performs other related projects and duties as assigned. REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: 1. Maintains clinical competency as required for the unit of assignment including but not limited to population-specific competencies and participates in annual skill competency verification. 2. Knowledge of federal and state regulations related to nursing applications and relative practice area. 3. Knowledge of computer hardware equipment and software applications relevant to work functions. 4. Ability to communicate effectively both verbally and in writing. 5. Ability to delegate, provide instruction and evaluate the work of LPN’s and unlicensed professional 6. Ability to maintain confidentiality of patient information. 7. Ability to exercise professional judgment, courtesy and tact in daily interactions with others. 8. Ability to establish and maintain effective working relationships with health care team members and diverse patient populations. 9. Ability to work in a fast-paced environment relevant to changing patient needs, including work with patients with acute, chronic, and complex disease processes, and those who are dying. 10. Ability to maintain regular and punctual attendance. EDUCATION AND EXPERIENCE: Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act. Certified Nursing Assistant

Summary Provides direct and indirect patient care under the direction of a Registered Nurse (RN) or physician. Assists patients with activities of daily living (ADLs), provides for personal care and emotional support and performs more complex clinical skills under the direction of an RN. Provides secretarial functions as needed for unit to function. *Commits to the KRMC mission, vision, values and goals and consistently demonstrates our core values. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. *Recognizes and reacts to emergency procedures per role and policies/procedures. *Performs and documents routine direct nursing care tasks according to identified age-specific needs. *Provides care appropriate to condition and age of patient, including pediatric, geriatric, and the general patient population. *Provides patient care that meets the psychosocial and physical needs of the patient within the CNA's scope of practice. Assists with patient admissions, discharge, and transfers. *Assists with patient care that is consistent with standards of care and clinical priorities determined by the nurse with the nature of the area/unit, patient care division policies and procedures, demonstrated competencies, and with consideration given to risk management and infection control practices. *Performs Activities of Daily Living (ADLs) for assigned patients and documents in the Electronic Medical Record (EMR). *Responsible for labeling and securing patient care belongings and ensuring that the patient room is safe and uncluttered. Provides surgical prep of patient with bath/shower then CHG, linen changes and other surgical prep requirements when necessary. Performs EKGs as ordered. Applies telemetry to patients under the direction of the nurse. *Distributes patient’s meals/water and assists with feeding of patients as needed. Answers patients’ call lights, anticipates patients’ needs and makes rounds of assigned patients. Assists with ambulation, lifting and positioning according to procedure, including the appropriate use of patient lift devices and use of good body mechanics. Assists with transporting patients, including at time of transfer and/or dismissal. *Assists patients with elimination needs and measures/records bodily elimination in medical record. Observes patient and reports complaints of pain or changes in mental and physical conditions to supervising nurse. Participates in patient care conferences. Documentation meets current standards and policies. Manages and operates equipment safely and correctly. *Cleans, stocks and cares for supplies and equipment in assigned work area. For inpatient areas: Provides morning care which may include bed bath, shower, provides foley peri care on patients with a foley Q shift. For inpatient areas: Performs general CNA care and tasks: oral hygiene, combing hair, back care, dressing patients, changing bed linen, cleaning over-bed table and bedside stand, straightening room, and other general care as necessary throughout the shift. For inpatient areas: Provides evening care which includes hands/face washing as needed, oral hygiene, back rubs, peri-care, freshening linen, cleaning over-bed tables, straightening room, and other general care needed. Summary GENERAL SUMMARY: Assists physicians with examinations and procedures to serve the medical needs of patients with various health conditions in a professional, confidential, and caring manner. Coordinates the flow of patients, phone calls, lab work, and nurse visits to facilitate quality patient care and customer service goals. Provides professional nursing care as prescribed by the physician and in compliance with the Nebraska Nurse Practice Act, any applicable licensure/certification requirement and organizational policies and procedures.
