Overview The Ambulatory Care Nurse (ACN) is a Registered Nurse responsible for providing nursing care management services to the assigned populations. The ACN utilizes a collaborative approach to anticipate, integrate, and coordinate the patient?s plan of care and maximize the ability to remain in a community setting. In collaboration with the physician, the nurse identifies at risk patient populations who require care management services. The ACN serves as a patient/family advocate and acts as a communication link between the physician, patient, family, and interdisciplinary health care team, as well as community resources. Job Description The RN @ The Cardinal at North Hills provides high quality nursing services to community residents based on each resident’s individual service plan and physician’s orders. You will monitor residents’ well-being in a general way and report any changes in condition Effectively supervise departmental operations on a day-to-day basis Assess residents to identify their physical, psycho social and environmental needs Communicate, identify, report and document problems Demonstrate knowledge of emergency procedures and respond appropriately Identify and recommend other disciplines needed for quality care of the resident Document evaluation f nursing interventions including resident response Assess resident and staff knowledge base of disease process, treatment regimen and/or medication administration and assist with care plans Provide resident care activities under the direction of the Assisted Living Director Assist with Medication Management Program Participate in the Assisted Living performance improvement process Present required new hire training and ongoing skills training for associates As well as perform other related duties as required What will I need to be successful in this role? One-year or more experience as a nurse in a senior care setting preferred Assisted living and/or dementia experience preferred Hands on experience with ports and Huber needles a plus Experience in wound care Hands on experience with Dialysis shunts a plus Special Requirements/Certifications I may need?
