Accurately documents all patient care per policy and payer requirements; communicates timely with physicians, patients, families, and agency staff to ensure coordinated, effective care. Participates in orientation and precepting of new staff; contributes to performance improvement, survey readiness, patient safety initiatives, and reducing avoidable hospitalizations.
