Provide basic nursing care such as checking blood pressure, inserting catheters, and helping comfort patients by performing tasks such as changing and dressing Document accurate and ongoing assessment of patient status: Document patient care, including nursing intervention, patient response to care provided, patient needs, problems, capabilities, limitations, and progress toward goals Communicate with RNs and physicians regarding patients’ needs Collaborate with other nurses and health team members to ensure patients’ wellbeing Teach patient/significant other appropriate health information in a timely manner and share written informational material, as applicable. Perform direct patient care, using established procedures, policies, and standards Prepare and administer medications according to policy and procedure. Observe and document patients’ responses to pertinent medications. Demonstrate ability to handle emergency situations in a prompt, precise, and professional manner. Admit, transfer, and discharge residents as required. Perform administrative duties such as completing medical forms, reports, evaluations, charting, etc., as necessary. Receive phone orders from physicians and record on the Physician’s Order Form. Chart all reports of accidents/incidents involving residents. Follow established procedures. Verify the identity of each resident before administering the medication/treatment. Ensure adequate medications, supplies, and equipment are available. Report needs to DON. Ensure narcotic records are accurate for your shift. Notify DON of all drug and narcotic discrepancies. Give a thorough, concise report at change of shift. Communicate pertinent information to the appropriate person in a timely manner. Performs all job responsibilities in accordance with safety and infection control policies and procedures, including thorough hand washing, use of disposable gloves where indicated and proper disposal of soiled materials. Follow established policies concerning exposure to blood/body fluids. Demonstrate correct and safe technique in the use of equipment, according to specific product information/instructions. Attend and participate in mandatory, and scheduled training and educational classes to maintain current certification as a licensed nurse. AGENCY - RN CASE MANAGER HOME HEALTH

Benefits: Weekly Pay, 403B retirement plan, Career advancement opportunities The RN Case manager will: Assesses patient/family learning needs, style and limitations and adjusts for delivery of information Establishes realistic goals and develops individualized plans of care in collaboration with the patient, family and members of the health care team Collaborates with patient/family and other healthcare providers and/or community resources with planning of care and discharge. Follows physician orders on all patients assigned in accordance with patient care policies Reconciles medications with patient and physician consistently Effectively manages assigned case load, within the team model of care delivery Establishes a daily work plan based upon patient/family priorities of service and total area needs. Promptly triages patient visits, messages, and phone calls according to priority and urgency. Demonstrates excellent physical assessment and care planning skills. Supervises HHA in accordance with state and federal requirements. Demonstrates knowledge of pharmacology and medication administration and reconciliation Communicates and collaborates with all disciplines in the home care setting on a regular basis or immediately if there are any critical needs or crisis interventions. Completes documentation accurately and timely Performs on-call responsibilities and on call services to patients and families assigned Participates in agency quality management and safety activities Participates in ongoing staff meetings and in-services Qualifications (Minimum qualifications will be considered required unless specifically stated otherwise) Experience: · Minimum of one (1) to two (2) years of experience in health care. · Minimum of 1 year experience in home health setting. Education and Training: · An associate’s degree in nursing is required. · A bachelor’s degree in nursing is preferred. (not needed) License, Certification & Registration: · Possesses and maintains current CPR Certification. · Current Massachusetts licensure: Registered Nurse. · Valid driver’s license and car insurance.
