Certified Medication Aide (CMA): 2pm-10pm

At this time, we are seeking a Medication Aide to join our team! Responsibilities This is the job of the Medication Aide to administer oral and external medications to residents under the direction and supervision of a Registered Nurse (RN) or a Licensed Vocational Nurse (LVN). • Follows current standards of hygiene, safety and infection control when handling, storing and disposing of medications. • Administers medications within 60 minutes of scheduled time. • Takes and records vital signs upon administration of medications when required. • Recognizes the resident's right to refuse medication. • Notifies the appropriate RN/LVN when making the decision not to administer medication or treatment. • Clarifies any order or treatment regimen believed to be inaccurate, ineffective or contraindicated by consulting with the appropriate licensed nurse (RN/LVN). • Secures medication cart. • Removes discontinued, outdated or deteriorated drugs from inventory. • Recognizes and reports unsafe conditions for medication provision. • Accurately documents medication provision. • Recognizes and reports general reactions to medications. • Recognizes the limitations of the Medication Aide role. Qualifications • High school diploma or equivalent. • Graduate of a state approved nursing assistant training program and passed a state approved competency examination. • Successful completion of a state approved Medication Aide course and is approved to administer oral and external medication. • Ability to perform medication administration in accordance with state regulations. • Strong communication skills Experience preferred but not required.

LVN/RN 2pm-10pm (Monday-Friday) Sign on Bonus!

Essential Requirements for LVN/RN Evaluates the physical, emotional, and social needs of the residents. Reviews residents care plans, assures plans are followed and documents resident medical records. Supervises care provided by nursing assistants assigned to the unit, providing intervention, education, and assignment adjustments. Collaborates with Director of Nursing and/or Assistant Directors of Nursing with any changes in care or issues that may arise. Prepares and administers medication and treatments as needed. Provides and/or assists charge nurses/treatment nurses with necessary wound care treatments as ordered. Consults with physicians. Receives and records orders as required. Checks crash cart, med carts, supply room to ensure proper supplies are in place and ready to use. Chart checks for new admissions and reviews orders as received. Provides assistance to other charge nurses and CNAs with turning, lifting, bathing, ADLs, etc. Ensures orders are implemented and carried out appropriately. What we require: Current Texas RN (Registered Nurse) or LVN (Licensed Vocational Nurse) licensure Current/active CPR certification required. Long-term care experience preferred but not required. License Vocational Nurse Registered Nurse 2PM 10PM

Job Type : Full Time DESCRIPTION Essental Responsibilities: Arrive to shift on time 2-10 Complete CNA’s assignment for the shift on the binder and on the board Review 24 hour report with charge nurse from the previous shift. Narcotic count (sign on shift must be completed at this time’s) all narcotic count sheets verified Room rounds with off going nurse ( make sure CNA’s are also doing rounds ) Review 24hr report for lab follow up , diagnostic follow ups and any pending orders Medication passes per time schedule, G-tubes, PO medications, PRN medications, Nebulizer treatments, PICC line medications and skin treatments (if any). Complete skin assessment assigned for shift Ensure all CNA’s documentation on ADLs Ensure all shower sheets are turned in to you and reviewed before end of shift Complete all charting, changes in condition and/or new orders or treatments received for specific resident in their charts. Point Click Care electronic documentation. Document schedule or cancelled doctors’ appointments in residents’ chart. (inform transportation of changes) For OOP residents please fill out appropriate form, and head to toe assessment completed. Complete incident report if any occur during shift Complete lab inquiries and noted on lab log for any new orders or any labs received (log them in).

Certified Nurse Aide 6a-2p

Shift is 6a-2p Job Duties: Provides patients' personal hygiene by assisting with baths, backrubs, shampoos, and shaves; assisting with travel to the bathroom; helping with showers and baths. Provides for activities of daily living by assisting with serving meals, feeding patients as necessary; ambulating, turning, and positioning patients; providing fresh water and nourishment between meals. Checking vital signs and weight; recording intake and output information. Provides patient comfort by utilizing resources and materials; transporting patients; answering patients' call lights and requests; reporting observations of the patient to nursing supervisor. Documents actions by completing forms, reports, logs, and records. Maintains work operations by following policies and procedures. Protects organization's value by keeping patient information confidential. Serves and protects the SNF community by adhering to professional standards, SNF policies and procedures, federal, state, and local requirements, and jcaho standards. Updates job knowledge by participating in educational opportunities; reading professional publications; participating in professional organizations; maintaining licensure.
