Certified Nursing Assistant (CNA) Responsibilities Include: Assist residents with Activities of Daily Living (Bathing, feeding, toileting, dressing, etc…) Assist residents with dental care, nail care, hair care, etc… Assure residents are kept dry to avoid breakdown. Bed bound residents need turned and positioned every couple hours. Obtain resident’s current weight when instructed. Serve food trays to residents and documenting food/fluid intake and output in resident’s medical record. Inventory and mark all residents possessions Make resident’s beds and keep room neat and tidy. Maintain confidentiality with resident information. Assure resident privacy at all times. Certified Nursing Assistant (CNA) Qualifications/Requirements: Must possess CNA certification in the state of FL. Excellent customer service skills. Previous experience in long term care preferred. Must be in good physical and mental health. Ability to work independently or part of a group. Effectively communicate with others. Pay based on experience $15-$16/hr. Registered Nurse

Registered Nurse (RN) NOW OFFERING DAILY PAY The Legacy at Boca Raton Rehabilitation and Nursing Center is looking for a talented and hard-working RN (Registered Nurse) to join our ever-growing team. Registered Nurse (RN) DUTIES AND RESPONSIBILITIES Assists in defining, maintaining, and interpreting standards of practice. Performs required skills (assessment, intervention) according to guidelines, policies, and procedures. Participates in interdisciplinary committees that influence or determine policies affecting nursing practice. Follow facility procedures/protocols for admitting residents transferring to hospitals/clinics/outpatient visits and completion of appropriate documentation based on the established guidelines. Makes rounds on all resident rooms and ensure that the L.P.N.’s, C.N.A.’s, and/or others under his/her supervision receive their resident reports, have their assignments and break times scheduled. Assist the L.P.N. in establishing routines that facilitate a high level of nursing practice. Receives and give reports from and to outgoing and incoming shifts. Count or ensure the counting of controlled drugs with outgoing and incoming shifts. Coordinates/Prepares MDS schedules for quarterly, annual and significant change assessments. Complete MDS Booklet as per schedule (Nursing Sections). Reviews and signs off on all completed MDS and RAP Summary. Develops care plan that demonstrate knowledge of criteria to ascertain resident specific needs based on health care problem(s). Makes needed revisions and updates to the care plans. Ensures that care plan is carried out which demonstrates familiarity with criteria to establish priorities specific to anticipated outcomes. Monitors the provision of care for the residents in his/her assigned unit. Interprets philosophy and objectives of nursing service to residents, families and others. Identifies resident/family learning needs based on the discharge care plan as applicable and carries out appropriate teaching required. Demonstrates ability to coordinate and collaborate with resident/family and other members of the comprehensive health care team. Monitors the administration and accurate recording of medications used. Investigates and follow through on medication errors. Notifies physicians of pertinent observations and reactions, incident and abnormal laboratory and x-ray results as required and documents same promptly. Transcribes physician orders timely and ensure orders are implemented as appropriate. Reports all resident, visitor and work-related accidents/injuries and demonstrates appropriate follow through by investigating, documenting and processing incident reports. Accepts guidance, supervision, and criticism if needed. Practices in compliance with the Nurse Practice Act and regulatory agencies regulations. Assures that a stock level of medications, medical supplies, equipment, etc. are maintained at adequate levels at all times. Assume the authority, responsibility and accountability of directing your nursing shifts. Other responsibilities as deemed necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator. MDS Coordinator (RN) The Legacy at Boca Raton Rehabilitation and Nursing Center Salary: Competitive Compensation Package Now Hiring: MDS Coordinator (RN) The Legacy at Boca Raton Rehabilitation and Nursing Center is seeking an experienced and detail-oriented Registered Nurse (RN) MDS Coordinator to join our clinical leadership team. The MDS Coordinator plays a critical role in ensuring the accuracy and timeliness of resident assessments, care planning, reimbursement optimization, and regulatory compliance. The ideal candidate will possess strong clinical assessment skills, extensive knowledge of the MDS process, and a commitment to delivering exceptional resident-centered care. Responsibilities Coordinate and oversee the completion of MDS assessments in accordance with federal and state regulations. Ensure timely and accurate completion, submission, and tracking of all MDS assessments. Collaborate with the interdisciplinary team to develop and update comprehensive resident care plans. Review clinical documentation to support accurate coding and reimbursement. Monitor Medicare and managed care residents to ensure appropriate reimbursement and regulatory compliance. Support survey readiness and participate in state and federal surveys as needed. Qualifications Current and active Florida Registered Nurse (RN) license required. Previous MDS Coordinator experience in a Skilled Nursing Facility required. Strong knowledge of MDS 3.0, RAI process, Medicare reimbursement, PDPM, and CMS regulations. Experience with care planning and interdisciplinary team coordination. Excellent assessment, organizational, and analytical skills. Strong communication and leadership abilities. Ability to manage multiple priorities and meet regulatory deadlines.
