Observe resident attendance, mood, behavior, and degree of involvement so that the facility activities & resident progress are evaluated, by noting, reporting and charting resident behavior. Assume the authority, responsibility & accountability of Activity Assistant/Aide. Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident’s response to the service. Develop & maintain a good working rapport with other departments within the facility to assure that daily activities can be performed without interruption. Create & maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility. Assist in preparing activity rooms/areas for scheduled activity programs. Recommend to your supervisor the equipment and supply needs of the activity department. Assist in cleaning up activity areas after completing activity functions. Follow the care plan in providing daily activities for resident. Purchase department supplies, clothing and personal items for the residents. Oversee residents engaged in scheduled activities to promote the physical, social, and mental well-being of residents by applying experience in activities and knowledge of diagnoses and conditions of residents. Assist in transporting residents to/from appointments, activity and social programs, etc., as necessary. Assist residents with bowel and bladder functions (i.e., take to bathroom, offer bed pan/urinal, protable commode, etc.). Assist non-diabetic residents with nail care including clipping, trimming, cleaning the fingers, fingernails, and toenails. Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. Change bed linens in residents' rooms. Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes. Collect specimens as instructed (i.e., urine, sputum, stools, etc.). Maintain intake and output records as instructed. Observe and report the presence of pressure areas and skin breakdowns to prevent pressure injuries. Perform restorative and rehabilitative procedures as instructed in accordance with the individualized plan of care. Provide non-pharmacological interventions for pain in accordance with the plan of care. Provide daily range of motion (ROM) exercises; record data as instructed. Assist nurses with admission transfer and discharge functions. Greet residents and escort them to their room; provide new resident orientation to the unit/facility. Keep the call light within easy reach of the residents. Follow established infection prevention and control procedures. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill and/or ability required. Must be a licensed Certified Nursing Assistant (CNA) in accordance with laws of the state. Experience in Long Term Care industry, preferred. CPR certification, preferred.
