What You Will Do (At a Glance) As an LPN at Mountain Top RHC, you will provide direct nursing care to residents under the guidance of an RN and physician. You will work closely with nursing assistants, communicate with families and providers, and help ensure residents are comfortable, safe, and well cared for. Transport / Van Driver (Per Diem) Location: Mountain Top Rehabilitation and Healthcare Center Job Type: Mountain Top Schedule: Flexible; includes occasional evenings and weekends Position Summary We are seeking a reliable and flexible Transport/Van Driver to provide safe and timely transportation for our residents to and from medical appointments and other destinations. This role is ideal for someone who enjoys working with older adults and can adapt to a changing schedule. Key Responsibilities Transport residents to and from scheduled appointments and outings Assist with transportation of new admissions as needed Maintain accurate transportation logs, including mileage and trip records Track and submit transportation-related costs as required Coordinate weekly transportation schedules with Nursing staff Prepare necessary documentation/packets for resident appointments Ensure facility vehicles are clean, safe, and compliant with regulations Assist other departments as needed Support resident care tasks when appropriate Maintain strict confidentiality in accordance with HIPAA regulations Qualifications High school diploma or equivalent required Valid PA driver’s license with an excellent driving record CPR and Basic First Aid certification required Previous experience working with older adults or in healthcare preferred Ability to read, write, and communicate effectively in English CNA Certification, preferred.
