MDS and Admissions Coordinator (RN) Highland Chateau Health + Rehabilitation Center Location: 2319 7th St. West, St. Paul, MN Job Type: Full-Time | Hourly Shift: Primarily Days | Monday-Friday The ideal candidate will assist with conducting comprehensive assessments of residents in a long-term care facility using the Minimum Data Set (MDS) system, while also managing the admissions process by evaluating potential residents' needs and coordinating their intake into the facility, ensuring compliance with all regulations and facilitating smooth transitions into care. Required Skills and Qualifications: Registered Nurse (RN) with current practice credentials in the State of Minnesota Thorough understanding of the MDS assessment process and Medicare/Medicaid regulations Excellent clinical assessment skills and ability to interpret resident needs Strong communication and interpersonal skills to interact with residents, families, and healthcare team members Detail-oriented with strong organizational skills to manage complex documentation and reporting requirements.
