Provide clean, routine wound care and skin assessments per physician orders. Monitor resident clinical status, record vitals, and report changes in condition to the attending physician and RN supervisor. Partner with and guide your assigned CNAs to ensure resident comfort, hygiene, and scheduled turn routines. Document shift assessments, medication passes, and clinical notes in our Electronic Health Record (EHR) system. LPN Qualifications License: Active, unencumbered Licensed Practical Nurse (LPN) license in the Commonwealth of Massachusetts (or active reciprocity / pending new graduate status). Certifications: Current CPR / BLS Certification.
