Position Summary: In coordination with the Health & Wellness Director the HWC will help manage the day-to-day healthcare operations of the community to ensure resident’s healthcare needs are met. This position is responsible for resident assessments, care planning support, medication and treatment management, clinical documentation, and ongoing communication with residents, families, physicians, and caregiving teams.
