Nurse Navigator

Position Purpose The Nurse Navigator is accountable for coordination of care and educational support to patients. This nurse also assists the department’s leadership in coordination and conducting specific programs that support clinical excellence. The goals of this position and clinical programs are to improve the care experience of the patient and their family, facilitate the work of the physicians and meet the needs of the hospital’s community in relation to education, screening, prevention, and treatment. Help coordinate care leading up to treatment and may be required to participate in discharge planning and telehealth appointments. The incumbent will communicate with the team on behalf of the patient. Position Purpose This position provides optimal patient care utilizing knowledge and skills within the context of the nursing process. The incumbent is accountable for individualized patient outcomes over an entire episode of care.
