We are seeking a n experienced MDS Registered Nurse (RN) to join our skilled nursing team Key Responsibilities: Complete MDS assessments in accordance with federal and state regulations and guidelines. Ensure accuracy and completeness of data. Resident Assessment : Conduct comprehensive assessments of residents, including physical, psychological, and social aspects. Utilize the MDS tool to capture resident status. Care Planning : Develop and update individualized care plans based on MDS assessment data. Coordinate care plan meetings with the interdisciplinary team. Regulatory Compliance : Ensure compliance with all applicable regulations and standards, including CMS guidelines and state-specific requirements. Interdisciplinary Collaboration : Work closely with other healthcare professionals, including physicians, therapists, social workers, and dietitians, to ensure a holistic approach to resident care. Requirements: State licensure as a Registered Nurse (RN) 2+ years of experience in a healthcare facility or related setting Strong understanding of skilled nursing standards of care Excellent critical thinking and communication skills
