Job Summary: We're seeking a motivated and experienced Assistant Care Coordinator to join our team. This full-time role requires a strong presence in the field, balancing administrative precision with hands-on support. As an ideal candidate, you'll have a minimum of 2+ years of experience in agency homecare or facility care, good availability including some weekends, and reliable transportation. Responsibilities: Field Support & Mentorship: Spend significant time in the field conducting caregiver introductions, performing on-site spot checks, and providing hands-on support to ensure care plans are being followed correctly. Shift Coverage: Provide direct client care and shift coverage as needed to ensure continuity of service during caregiver transitions or emergency call-outs. Client Onboarding Assistance: Assist the leadership team with the transition from lead to active client, ensuring all "Post-Orientation" protocols are met. Quality Assurance: Monitor caregiver performance and client satisfaction, acting as a bridge between the front-line staff and the office leadership. Administrative Coordination: Utilize Axis Care and Google Workspace to update care notes, track compliance, and assist with the seamless flow of documentation. Team Collaboration: Work closely with the Care Coordinator and Director of Nursing to identify staffing needs or clinical changes that require immediate attention. Requirements: Experience: Must have an active RI CNA license with previous experience in a homecare or healthcare setting. Experience in a lead caregiver or supervisory role is highly preferred. Communication: Strong interpersonal skills; you must be comfortable coaching staff and communicating professionally with families. Problem-Solving: The ability to think on your feet in the field and resolve conflicts or scheduling hurdles as they arise. Mobility: Must have a valid driver's license and reliable transportation to travel across our service areas in Northwest RI and the Blackstone Valley. Tech Literacy: Proficiency with mobile apps and cloud-based software for documentation and scheduling.
