Certified Nursing Assistant

Position Overview: The Certified Nursing Assistant (CNA) is a licensed member of the home care team. The CNA is responsible for the basic daily care of the client. CNAs will help maintain care records for the client and communicate with other members of the care team. CNAs report to the Administrator and or Alternate Administrator. Essential Functions: To enable patients to stay in their homes by monitoring and recording the client’s physical and mental condition. To maintain a safe, secure and healthy client environment by following necessary standards and procedures. To provide client support and personal services. To protect the home health care company by adhering to professional standards, home care policies and procedures, federal, state, and local requirements. To follow HIPAA guidelines regarding all client PHI. Responsibility of a CNA: Monitors client condition by observing physical and mental conditions, intake/output, and reports any changes to the Administrator and or Alternate Administrator. Provides personal services such as bathing, dressing and grooming. Teaches clients how to use cane or walker, special utensils to eat, techniques and equipment for personal hygiene. Assist in planning and preparing meals and snacks according to specific dietary requirements as outlined in the client’s care plan. If necessary, the CNA will help with client feedings. Observe conditions of the client’s residence. Report on any changes to the following: Client’s physical behavior Client’s mental behavior Potentially unsafe environment changes to the supervising Administrator and or Alternate Administrator. Enhances service reputation by accepting ownership for accomplishing new and different requests. Explains the necessary procedure to the client prior to performing it. Light housekeeping duties as needed. Maintains continuity of client care by documenting and communicating with other caregivers in the client’s care plan (book) and in the system. Participate in in-services and training as required. Skills/Qualifications: Independence, flexibility, good judgment, strong verbal and written communication, physiological knowledge, nutrition, ability to maintain a good client relationship, listening skills, time management, creates a safe environment, health promotion and maintenance, nursing skills, client service oriented, and emotional control. Valid driver’s license and reliable transportation. Characteristics: Dependable, team player, efficient, personable, works well with others (especially elderly), flexible, compassionate. Education/Skills Requirements: Current CNA certification. Must maintain current credentials: First Aid, TB test, and in-services. Background check and complete a drug test and randomly thereafter. Physically able to perform duties of the position. General nutrition knowledge necessary for meal planning. Good oral and written communication skills. ESSENTIAL PHYSICAL DEMANDS Bending, stooping, walking and general physical demands of light housekeeping. Standing for prolonged periods. Able to lift certain lbs.

Registered Nurse Field Supervisor

In this role, the RN will supervise, mentor, and support Certified Nursing Assistants (CNAs), Medication Aides (MAs), and Companions, in line with the policies and procedures set by A Place at Home and the state of Texas The Registered nurse reports to the administrator and is expected to assist A Place At Home-River Oaks with nursing assessments, employee training, and any necessary administrative tasks to ensure that A Place At Home- River Oaks remains compliant with the Texas Department of Health. The registered nurse ensures quality service provision and appropriate supervision. The registered nurse is responsible for working with other disciplines to ensure client needs are met, which includes, but is not limited to, the assessment process, the creation and maintenance of the plan of care, the coordination of services, and follow-up assessments within 90 days of the initial assessment and as needed in a change of client condition. Essential Job Responsibilities: • Provides excellent customer service skills and ability to deal tactfully with clients, staff, and community. • If directed by the administrator, makes the initial evaluation visit and regularly re-evaluates the client's needs. • Initiates the plan of care and makes necessary revisions, keeping the administrator updated on the revisions. • Informs the administrator, physician, and other relevant personnel of changes in the client's condition and needs. • Implements and ensures compliance with agency policies and procedures. • Assists the administrator with assessing the competency of caregiver staff to promote quality, continuity, and safety of client care. • Supervises, educates, and coaches other personnel as directed by the administrator. • Ensures proper maintenance of records in compliance with local, state, and federal laws. • Ensures compliance with all local, state, and federal laws. • Identifies and participates in agency performance improvement activities. • Stays informed of changes in the field of home care and overall healthcare to share with appropriate personnel. • Promotes home care referrals in the healthcare community. • Knowledge of HIPAA regulations ensuring confidentiality of patient information at all times. Qualifications: Active RN License (Required) Clinical experience or home care experience, preferred. Minimum of one (1) year supervising experience, preferred. Case management skills, preferred.
