What You'll Do as an RN MDS Coordinator (RNAC): Complete and coordinate the RAI/MDS assessment process for assigned residents, ensuring assessments, MDS documentation, CAAs, and related information are accurate, complete, and submitted within required timeframes. Develop comprehensive, individualized resident care plans upon admission, annually, and following significant changes, with a focus on achieving the best possible resident outcomes. Coordinate with the interdisciplinary team (IDT) by conducting resident and staff interviews, communicating assessment schedules, monitoring required documentation, and leading care plan or MDS-related meetings as assigned.
