This role is crucial in assessing patient needs, coordinating care, and ensuring effective communication among all parties involved in patient care. Responsibilities: As a Registered Nurse Case Manager, you will: Coordinate DME Services : Oversee the coordination of durable medical equipment , supplies, and related services by collaborating with physicians, vendors, therapists, and internal departments to support patient care needs. Obtain and Manage Physician Orders: Communicate with MD offices and healthcare providers to obtain signed orders, letters of medical necessity, supporting documentation, and any additional clinical information required for equipment and supply processing and approval.
