Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. Engages in formal and informal peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. Contributes to quality and safety practices and required compliance measures. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. Description The Home Health Nurse Manager is responsible for the management and oversight of clinical staff for the licensed home care agency and nursing staff supporting home infusion services under the guidance of the Director of Patient Services, S/he ensures that personnel issues are managed in compliance with Department of Health (DOH) regulations and contractual agreements. S/he works in collaboration with clinical leadership, administrative leadership, providers of care and referrers to ensure that the service maintains the highest standards of patient care and is responsive to the needs of the patient and family and the organization. Responsibilities Patient Experience 1. Supervises all clinical staff of Mount Sinai Home Health LLC home care agency and nursing staff supporting home infusion services. 2. Oversees the agency orientation for all newly hired clinical staff including introduction to agency policy and procedures, clinical skills training and assessment, documentation and EMR requirements and competency evaluations. 3. Ensures clinical staff adherence with written policies and procedures 4. Performs field supervision visits with all clinical staff on an annual basis at a minimum and remediation supervision visits as needed. 5. Participates in the recruitment and selection of personnel. 7. Oversees the maintenance of patient clinical records and clinical staff personnel files. 8. LPNs working exclusively in chronic outpatient hemodialysis settings may, under the direct supervision of a Registered Professional Nurse (RN) and after completing an approved hemodialysis training and competency program, perform dialysis-related tasks as permitted by the New York State Education Department (NYSED) Office of the Professions and 10 NYCRR §700.4(b). See Section D for more details regarding role of LPNs in the Outpatient Hemodialysis setting. This exception does not permit independent practice or unsupervised access to central venous lines. 14. Assists in patient assessment after the RN has completed the initial patient assessment, including pain management. 15. Carries out medical and nursing orders. Cannot accept verbal orders or carry out RN standing orders. 16. Makes, reports and records observations on assigned patients within the EMR. 17. Observes and reports any signs and symptoms of change in a patient’s condition to the appropriate person. 18. LPNs in the Outpatient Hemodialysis Setting An LPN can practice in the chronic hemodialysis outpatient department under the direct supervision of an RN. The LPN will participate in educational programs on a regular basis, including initial orientation and continuing in service training to care for patients with central venous access devices (CVAD). The training should include a written examination and competency demonstration. A Hemodialysis LPN may: • Administer bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first dose administration which is administered by an RN. The RN will access and care for patient’s initial catheter use. Subsequently, and following the MSHS CVAD care policy, the LPN can: • Identify and set up supplies necessary for accessing a CVAD. • Initiate treatment with a CVAD. • Draw blood specimens from a CVAD. • Perform CVAD dressing change and exit site care. • Monitor and adjust flow rates of central venous catheter access device/line. • Intervene if there is a catheter/blood line disconnection or other complications associated with a CVAD. • Provide CVAD care at the end of dialysis treatment. Qualifications A. Completion of educational program for licensure as a LPN. B. Successful completion of required New York State Department of Health LPN IV Therapy Initial Program for the demonstration of competence in the performance of intravenous therapy/blood administration procedures as part of Centralized Orientation. C. Successful completion of annual unit-based review & update program in procedures associated with intravenous therapy.
