BRIEF JOB DESCRIPTION: The Health Assistant is responsible for direct and indirect nursing care to enhance the well being, academic success, and life-long achievements of students. The Health Assistant facilitates positive student responses to normal development, promotes health and safety, intervenes with actual and potential health problems, and actively collaborates with others to build student and family capacity for adaptation, self-management, self advocacy, and learning. PRIMARY FUNCTIONS OF THE JOB: Assess and evaluate students' medical history, including social, behavioral, psychological and familial concerns. Utilizing evidenced based practice, identify need for appropriate nursing services in the school setting Initiate a plan of care with student and family input to best meet needs of individual student. Implement plan of care with consultation with school staff and community providers as necessary. Evaluate effectiveness of interventions and revise plan as necessary. Provides emergency care for all students and staff on school premises in the event of injury or sudden illness. Administer prescribed and approved medication to students with parent/guardian and physician consent. Perform mandated screenings in vision, hearing, scoliosis, and height and weight as deemed appropriate by state and city departments of public health Documents pertinent information in student records in accordance with Standards of Practice and safeguards documentation utilizing HIPPA and FERPA regulations Conducts home visits to initiate or follow up on the health care needs of students Prepare regular written reports for supervisor, MDPH and other relevant agencies. Notify appropriate individuals, ie parents, staff, physicians of immediate and potential health related concerns. Provides information about health and disease prevention in cooperation with health education programs within Springfield Public Schools Promotes healthy programs in collaboration with coalitions, associations, CDC and Department of Public Health Acts as a resource in health education to students and families and refers to appropriate help agencies when necessary Report to work as scheduled and conducts self in professional manner. Location: Main Hospital Bldg - 7th Floor **NIGHT SHIFT** Full Time Position Primary Purpose The RN is a licensed professional who uses the Parkland nursing professional practice model to coordinate patient care delivery, that focuses on a particular patient population(s)/specific health conditions, by the health care team. Using the nursing process, the RN assesses the patient, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response. The RN utilizes knowledge of patient needs and the healthcare environment to assist patients to transition through the healthcare encounter without any preventable complications or delays. The RN delegates interventions to health care personnel based on the Texas Nursing Practice Act, each patient's condition and the competencies of the employee. Minimum Specifications Education Must be a graduate from an accredited school of nursing. Prefer Bachelor of Science in Nursing. Experience Must have two years of nursing experience. Must demonstrate clinical proficiency performing ultrasound guided PICC insertions, maintaining aseptic technique while utilizing measures that prevent patient discomfort and trauma related to the procedure. Must demonstrate high level skills in critical thinking and assessment related to vascular access devices. Responsibilities Serves as a patient advocate by focusing on patient needs, patient rights, confidentiality, and religious and cultural preferences. Serves as a resource for patients and other healthcare team members for specific clinical patient care issues. Provides patient care educational information and training to patient and family members for those requiring care in the Radiology Department. May participate in community outreach activities (such as health fairs, and community collaborations of outreach clinics) as assigned. Maintains and implements knowledge of applicable rules, regulations, policies, laws and guidelines that impact or govern Radiology nursing practice, and ensures that Parkland Hospital is in compliance. Develops effective internal controls that promote adherence to applicable state/federal laws and the program requirements of accreditation agencies and federal, state, and private health plans. Maintains and updates knowledge and appropriate utilization of sanitation, infection control, safety, supplies, equipment, and materials. Performs appropriate patient assessments including continuous surveillance. Participates consistently in Universal Protocol and site marking identification. Recognizes changes in a patient's condition and provides appropriate interventions as necessary and ordered. Consults with other team members as required, notifies providers to report changes in patient condition and implements orders in a timely manner. Exercises independent judgment in providing and prioritizing safe nursing care to patients. Must be able to manage a radiology patient with varied medical conditions. Must provide safe medication administration using the patient's six (6) rights, provide maintenance and management of IV access. Must be able to provide emergency resuscitation care of an adult. Administers sedation and analgesia. Physiologically monitors patient during procedures and uses monitor to record physiological status and documents accordingly. Assesses and monitors patient condition including vital sign monitoring and recognition of ECG changes, and initiates appropriate nursing interventions. Completes appropriate documentation. Maintains accurate and timely entries in patient care records and hospital documents. Ensures that all records and documents meet the standards, goals, and objectives of the Radiology department and Parkland. Works with other staff in providing direction to non-nursing staff. Participates in information exchanges with nursing and other Parkland staff that support the standards, mission, goals and objectives of the Radiology department and Parkland. Provides appropriate hand off any time care is relinquished. Seeks and utilizes direction from more experienced staff and team members as needed. Serves as Charge Nurse as needed. Serves as a preceptor or trainer to newer nursing and clinical staff members as well as student trainees. Prepares and conducts an in-service educational program for the Radiology Nursing Department at least once per year. Identifies ways to improve work processes and patient and customer satisfaction. Makes recommendations to supervisor, implements and monitors results as appropriate in support of the overall goals of the Radiology department and Parkland. May serve on various unit and interdisciplinary committees as opportunities present. Proactively anticipates and responds to the needs of patients, their families, physicians, and coworkers. Demonstrates an energetic and positive approach to work. Maintains a positive working relationship with all Parkland internal and external customers, employees, etc. Demonstrates a sense of ownership and pays attention to details of the job to ensure all mandatory obligations are met. PURPOSE AND SCOPE: The professional registered nurse Inpatient Services RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). Documentation Nursing process is accurately and concisely documented in the patient’s electronic medical record reflecting the plan of care as implemented by the patient, family, and multidisciplinary team. Documents information in a timely manner, according to hospital policy, to include the plan of care, accurate updates, teaching and the patients’ and family response to care. Leadership and Professional Development Demonstrates leadership skills such as the ability to motivate, educate, delegate, and mentor peers and other members of the healthcare team while effectively managing the care of patients and family members. Assumes responsibility and accountability for professional growth and development. Contributes to the professional development of peers, colleagues, and others. Participates in 75% of staff meetings and 75% of council meetings as appropriate. Participates in unit based activities/initiatives for performance improvement, evidence based projects, and research as needed. Participates in council activities and task forces to improve competencies of self, co-workers, and staff members related to patient care. Supports self and peers in professional growth and development utilizing resources and opportunities within and outside of The Christ Hospital. Participates in self and peer review activities to include the positive recognition of peers and staff on a regular basis. Attendance Follows the time and attendance policy; reports to work on time, maintains a good attendance record, has badge and uses it to clock in and out, makes requests for tie off as far in advance as possible, etc. Qualifications KNOWLEDGE AND SKILL: Please describe any specialized knowledge or skills, which are REQUIRED to perform the position duties. EDUCATION: TCH does hire AD and Diploma nurses; however all nurses hired after January 2010 must receive their BSN within 5 years of the date of hire. RN’s who were employed at TCH prior to January 2010, and have more than 20 years of experience as an RN are permitted to pursue their BSN on a voluntary basis. A MSN or DNP are not required but supersede the requirement for the BSN. YEARS OF EXPERIENCE: None required REQUIRED SKILLS AND KNOWLEDGE: The RN demonstrates the knowledge, abilities, and skills to provide age and culturally specific patient care and education. The RN effectively communicates with peers, utilizes appropriate channels of communication and maintains absolute confidentiality. The RN maintains competence and demonstrates evidence of continuing professional growth. The RN demonstrates the ability to accept and implement change and the ability to work in a culturally diverse setting. LICENSES & CERTIFICATIONS: The RN holds a current licensure to practice nursing in Ohio. Membership in professional organizations is desirable. BLS certification is required prior to patient contact. ACLS is required within 6 months of hire in step-down and ICU areas. Certification in area of clinical specialty is expected within five years of eligibility.
