Responsibilities The RN Case Manager assesses the social, psychological, cultural, environmental and financial situations concerning the patient and family. He/she is a patient/family advocate, promoting rights and dignity and striving to involve the patient/family in aspects of care as appropriate. The RN Case Manager also facilitates the acute, rehabilitative, and long-term discharge processes to provide for an optimal age-specific continuum of care for the populations listed below. Accountable for the care of patients assigned to them in accordance with the Nurse Practice Act and Policies and Procedures of BSA. Qualifications Job Requirements: Graduate of Accredited School of Nursing Current License as RN in the State of Texas BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care.
