DEPARTMENT : CMA/LPN/RN - Cameron Family Medicine - West Maumee JOB TITLE: CERTIFIED MEDICAL ASSISTANT (CMA), LICENSED PRACTICAL NURSE (LPN), REGISTERED NURSE (RN) SHIFT: PRN Part-time, As Needed , Days. No weekends or holidays QUALIFICATIONS Graduate of an accredited nursing program or medical assistant program. Current LPN, or RN license in the State of Indiana. Certified medical assistant or registered medical assistant. Has general knowledge of nursing theory and practice. Has been educated in basic pharmacology and the administration of medications. CPR provider certification within 6 months. JOB SUMMARY Provides nursing care to patients as directed by the provider utilizing the nursing process. Performs patient access functions. ESSENTIAL JOB FUNCTIONS AND PERFORMANCE STANDARDS Exemplifies good customer service. Regular reliable attendance. Effectively communicates and gets along well with others. Demonstrates proficient use of email. Accomplishes goals set for self and by Office Manager. Completes patient care assignments as directed by provider and clinical protocol. Reports significant findings to the provider. Prepares clinic areas for patient care at the beginning of the shift. Secures clinic areas nightly. Proficient in EPIC including retrieving patient records, entering patient data (e-form) and locking record. Greets patients on arrival. Registers and enters patient data into the Health Information system. Scans insurance card information. Admits patients, taking brief history and vitals. Performs procedure set-up and assists provider as indicated. Discharges patient with prescriptions/instructions and documents in patient’s chart. Maintains adequate medical supply and medication inventory. Schedules surgeries or referrals as ordered, attain prior authorizations for surgeries when needed, provides patient education and instruction information. Familiar with policies in Policystat, including nursing, registration and disaster policies. Performs other duties as assigned by the Office Manager.

CMA/RN/LPN, Current BLS or ability to obtain within 3 months of hire. OB/GYN experience is a plus but not required. Position consists of both clerical and clinical duties.

DEPARTMENT: OP IV Clinic JOB TITLE: Registered Nurse - OPIV Clinic SHIFT: FTE 0.4 (32 hours bi-weekly) Part-Time Summary/Objective The Registered Nurse is responsible for day-to-day patient care operations and will provide direct patient care according to patient and unit specific needs, and is responsible for assessing, planning, prioritizing, organizing, administering, and evaluating patient care. Essential Functions Assures the accurate and timely delivery and documentation of patient care, including acknowledgement of physician’s orders, administration of treatments, medications, and related procedures. Completes an accurate and complete patient assessment and responds accordingly. Provides patient education at discharge and throughout stay Maintains efficient patient throughput Ensures compliance with hospital policies, procedures, and quality assurance standards Serves as a clinical resource to staff members and assists with daily tasks, decision making and problem solving. Follows standard precaution guidelines Uses AIDET format, establishes, maintains, and promotes among staff members positive working relationships with internal and external customers for the effective resolution of concerns in area of responsibility and in support of hospital goals and objectives. Communication: effectively communicates with patients, staff, physicians, and visitors verbally or in written documentation. Completes and documents in the electronic medical record in a thorough and accurate manner as defined by unit policy. Assesses the patient/family education needs and most effective methods. Must be proficient in medication administration, including IV therapy, as evidenced by lack of medication errors. Reports medication errors. Participation in unit specific initiatives such as education offerings, unit meetings, quality and process improvement. Qualifications Graduate of accredited professional nursing program, BSN preferred Current RN license in Indiana Experience Prior experience preferred but not required Skills/Competencies Strong analytical and organizational skills to prepare and maintain various documentation in compliance with hospital policies and procedures. Strong communication skills promoting teamwork and group achievement as well as one on one communication with patients. Graduate of an accredited nursing program. 2. Current license in the State of Indiana. 3. Prefer two years’ experience in ER/Critical care nursing. 4. BLS provider certification within 6 months. 5. Certified in ACLS within one year from date of hire. 6. Certified in PALS within one year of hire. 7. Certified in TNCC within two years of hire if statuses in ED. 8. Certification in Neonatal Resuscitation within one year of hire. 9. Certification in TB course within one year of hire. JOB SUMMARY Provides nursing care utilizing the Nursing Process. Demonstrates proficiency of Order Communications computer system. 14. Familiar with fire, tornado, bomb, disaster and Haz Mat procedures. 15. Follows Standard Precaution guidelines. Answers EMS radio. 5. Assigns break periods and meals. 6. Coordinates admissions and transfers. 7. Handles patient calls as needed. 8. Notes and initials physicians' orders. 9. Discharges patients as needed. 10. Notifies supervisor if additional help is needed. 11. Ensures professionalism within the department. 12. Handles problems that arise.
