We seek an experienced, compassionate, skilled Licensed Vocational Nurse (LVN) to join our team at a Skilled Nursing Facility. As an LVN, you will be responsible for providing direct nursing care to our residents under the direction of a registered nurse (RN) and ensuring that the facility meets all applicable regulations and standards. Responsibilities: Provide direct nursing care to residents, including administering medications and treatments, monitoring vital signs, and assessing and documenting changes in their condition. Assist with developing and implementing individualized care plans based on residents' needs and goals. Coordinate care with other healthcare team members, including physicians, therapists, and other healthcare professionals. Ensure that all documentation and charting are accurate, complete, and up-to-date. Assist with activities of daily living (ADLs) as needed. Educate residents and families about their healthcare needs and treatments. Respond to emergencies and provide first aid as needed. Adhere to all policies, procedures, and regulations governing nursing care in the facility. Qualifications: Valid and current LVN license in the state of California Graduate of an accredited nursing program Minimum of 1-year experience in a skilled nursing facility, acute care setting, or in-patient rehabilitation, or in-patient behavioral health setting Strong clinical skills, including assessment, planning, and implementation of nursing care Excellent communication and interpersonal skills Ability to work independently and as part of a team Strong critical thinking and problem-solving skills We are committed to providing high-quality care to our residents and believe our staff is our most valuable asset. Colonial Gardens is a custom-designed skilled nursing facility located in Pico Rivera, California. With 99 beds available, we deliver personalized and compassionate care to ensure our community members experience stability, fulfillment, and well-being. At this time we are seeking compassionate and dedicated CNAs for (NOC ) Shift. 10:30pm - 07:00am. Part-time positions are available across all shifts. We provide competitive wages and benefits to the right candidates. Responsibilities: Assist residents with daily living activities such as bathing, dressing, grooming, and toileting. Transfer residents to and from activities and meals as outlined in their individual service plans. Serve meals to residents in the dining room or their apartments. Record and report any changes in residents' eating habits to a supervisor. Uphold quality services in compliance with company, state, and federal regulations. Qualifications: Current CA nursing assistant certification. Current CPR and first aid certifications are a plus.
