Family Medicine Nurse Practitioner

Pay Range: $60.00 - $80.00 per hour Position Details Eisner Health is seeking an experienced Family Medicine Nurse Practitioner who specializes in Adults, Pediatrics and Family Practice. The Family Medicine Nurse Practitioner will be responsible for working collaboratively with physicians and other health care professionals to provide primary health care to acute and chronically stable patients. Including, obtaining histories, perform physical examinations, diagnoses, treat and monitor chronic diseases, order and interpret diagnostic studies, prescribes and select medications, immunizations and other treatments. The ideal candidate will possess excellent interpersonal skills to interact effectively with professionals, administrative staff, clients and representatives of community agencies. This may include but is not limited to the following responsibilities: Educates and counsels patients to promote positive health behaviors and disease prevention. Performs and completes routine physical and follow-up exams. Diagnoses and treats acute and chronic medical problems. Orders diagnostic tests, x-rays, therapeutic injections and immunizations as determined by the symptoms or illness of the patient. Work closely with clinical staff in coordinating and obtaining needed services for patients and families. Keeps currently dictated or written medical records on and records all pertinent information regarding patient history, physical diagnoses and treatments accurately. Refers patients to specialists in accordance with EISNER’s referral policy when deemed necessary. Uses, protects, and discloses EISNER patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards. Consults with attending physician as necessary. Performs additional duties as assigned. Requirements And Qualifications Master’s Degree Nursing Minimum 3 years of broad spectrum family medicine experience Proficient in English and Spanish highly preferred Current CPR Certification Current NP License required Experience in Adult, Pediatrics and/or Family Practice Must be able to work in interdisciplinary teams Benefits (for employees working 32 hours per week or more): $500 per year CME Stipend & 5 days of paid CME time per year License & DEA reimbursement Low-cost Medical, Dental, and Vision Insurance with PPO & HMO options Robust Paid Time Off Program (23 days accrued per year for FTEs), PTO accrues upon hire. Take the next step in your career and join the Eisner Health team! Position Details Eisner Health is seeking a Pediatric Nurse Practitioner who will provide services to underserved communities. A successful candidate will be responsible for providing direct patient care, multidisciplinary interaction with support services, and other clinical and administrative responsibilities. Utilizing the guidelines set forth by the American Academy of Pediatrics and the Department of Health’s Child Health and Disability Prevention Program (CHDP), the nurse practitioner shall provide primary care to pediatric patients ages 0-18. Job Duties The nurse practitioner as the Primary Care Provider (PCP) who has been assigned to the patient, shall see the patient at the earliest opportunity following, whenever possible, specified HMO guidelines for initial evaluation and subsequent visits To take care of the patient's immediate needs. To undertake medical evaluation, including history and physical examination, laboratory, X-ray, referrals, or other diagnostic studies. To evaluate the patient and develop a problem list. To develop treatment objectives and plans. To initiate treatment orders and procedures. To work closely with other staff members in coordinating and obtaining needed services for patients To educate and counsel patients and their parents to promote good health and disease prevention To use and disclose patients’ protected health information only in accordance with the Health Insurance Portability and Accountability Act (HIPPA) standards. To practice within the confines of the Center’s established treatment protocols, and as directed by the supervising physician. To consult with the physician supervisor whenever any question arises as to diagnostic evaluation or medical treatment of any patient The PNP will make every reasonable effort to follow through personally with the patients she/he evaluated initially. Patients will be asked to return for further services only as often as necessary and by appointment whenever possible. A visit form will be recorded in the EHR system, signed electronically, for each patient visit To reflect change in a patient’s condition To indicate any diagnostic studies performed, and their results when they become available To indicate changes in patient’s active problems, diagnosis or treatments To indicate the logic of diagnostic evaluations and treatment plans To reflect any pertinent attitudes or behaviors on the part of the patient or their family towards any aspect of patient care To easily allow continuity of care by other providers Specialty clinic services shall be provided if medically necessary. Recommendations made by subspecialists will be integrated into the patient treatment plan. The patient shall be informed by the PNP as to the general nature of his/her medical problem, the general prognosis and the nature and purpose of the treatment and procedures that are contemplated. Appropriate instructions in self- care shall be given by the PNP to the patient and documented in the patient's medical records. A second opinion referral will be generated if requested by the patient’s family, and deemed medically reasonable. A chronic problem list will be generated and maintained as current as possible. Items in the PAQ will be addressed on a regular basis. All medical providers are expected to promote a cohesive, cooperative working relationship with fellow providers which may include helping with excess patient loads and staying after the scheduled working hours. All providers are expected to maintain excellent professionalism when dealing with patients. Qualifications License to practice as a nurse practitioner in the State of California. Board certified or board eligible in field of Pediatrics. At least 2 years clinical experience as a nurse practitioner in area of specialty. Knowledge and experience in managed care preferred. Effective communication and interpersonal skills with children and families. Proficiency in Spanish is highly preferred Maintain CPR certification.
