CNA Scheduler Supervisor

$21 - $25 / hour
SUMMARY: The CNA Supervisor position is responsible for ensuring appropriate staffing for our facility while maintaining the staffing regulations. Additionally, the incumbent will communicate effectively with all staff, ensuring to track Nursing employees concerns. The incumbent will also maintain and order medical supplies as needed for the nursing department. ESSENTIAL DUTIES AND RESPONSIBILITIES: •Maintain schedule for Certified Nursing Assistants (C.N.A.s) and Nurses (RNs and LPNs) •Reduce overtime to the best of his/her ability in the nursing department in conjunction with the DON/Assistant Director of Nursing (ADON) and Human Resources Director •Assist in hiring new C.N.A.s and Nurses on an as needed basis communicating with the DON/ADON and HR Director •Maintain accurate time records for the nursing department; ensuring to monitor attendance (Call-Offs, Tardies, and No Call, No Shows) •Responsible for timely ordering and stocking of medical supplies for the facility •Ensure that all nurses stations are stocked with the necessary supplies •Organize nurse's stations on a weekly basis •Organize all rented equipment (i.e. beds, wheelchairs, etc.,) and send back when not in use •Assist with schedule and transport of residents and assist with scheduling for escort needs •Other duties as assigned QUALIFICATIONS: •Must possess, as a minimum, a high-school diploma or its equivalent and be a current CNA •Previous scheduling experience desired •Strong oral and written communication skills •Proficient with Microsoft Office (Word, Excel, PowerPoint, Outlook) •Ability to multi-task, prioritize and work under pressure •Professional appearance and demeanor •Ability to be resourceful, tactful, and proactive in dealing with any issues that may arise PHYSICAL DEMANDS: The physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job. MDS Coordinator

$80,000 - $90,000 / year
Must be an RN or LPN SUMMARY: The MDS Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for care and services provided within the Facility. Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission. He/she will integrate nursing, dietary, social recreation, restorative, rehabilitation and physician services to ensure appropriate assessment and reimbursement. ESSENTIAL DUTIES AND RESPONSIBILITIES: • Assesses and determines the health status and level of care of all new admissions. • Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change. • Communicates level of care for new resident to all disciplines. • Coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission to facility according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal, state and medical standards. • Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident's stay. • Responsible for the data entry function to assure accurate data entry and electronic submission of MDS assessments. • Verifies electronic submissions of MDS, performs corrections when necessary and maintains appropriate records. • Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal and state standards. • Schedules and conducts resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference. • Assists disciplines in formulating and revising care plans. Ensures that resident's present/potential problems are identified and prioritized; realistic goals are established and nursing intervention is appropriate. • Evaluates resident care plans for comprehensiveness and individuality. • Assesses the achievement or lack of achievement of desired outcomes. Ensures that resident's care plan is reassessed and revised appropriately. • Responsible for all level of care changes within the facility. Notifies all departments when a level of care change has been made. • Generates appropriate forms to complete level of acuity and changes. Transmits forms to the appropriate agency for processing as required by state law.

SUMMARY: The primary purpose of this job position is to assist in the planning, implementation, and evaluation of recreational, social, intellectual, emotional and spiritual programs, in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assist in planning, developing, organizing, implementing & evaluating the activity programs & quality assurance program in this facility for the Activities Department. Observe resident attendance, mood, behavior, and degree of involvement so that the facility activities & resident progress are evaluated, by noting, reporting and charting resident behavior. Assume the authority, responsibility & accountability of Activity Assistant/Aide. Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident’s response to the service. Develop & maintain a good working rapport with other departments within the facility to assure that daily activities can be performed without interruption. Create & maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility. Assist in preparing activity rooms/areas for scheduled activity programs. Recommend to your supervisor the equipment and supply needs of the activity department. Assist in cleaning up activity areas after completing activity functions. Follow the care plan in providing daily activities for resident. Maintain the confidentiality of all resident care information including protected health information & follow facility guidelines in reporting non-compliance with said guidelines & treat residents with dignity at all times. Oversee residents engaged in scheduled activities to promote the physical, social, and mental well-being of residents by applying experience in activities and knowledge of diagnoses and conditions of residents. Assists in-room residents by visiting with them, writing letters, running errands, and perform assistance with crafts, projects, etc., as necessary. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required. Experience in Long Term Care Activities Department is preferred. Must have patience, tact, cheerful disposition & enthusiasm, as well as be willing to handle residents based on whatever maturity level at which they are currently functioning. Must be able to read, write, speak, and understand the English language.
