In this role you will participate in assessment, implementing, and documenting resident care as ordered by the physician. You will also direct and oversee the functions of CNAs and CNA Med Techs. What you’ll do: Assist in the day-to-day operation and administration of Assisted Living and Memory Care units. Assist and support the clinical care coordinator and AL care coordinator Oversee and direct the care activities of CNAs and CNA Med Techs. Provide direct patient care. What you’ll need NC LPN License 3-5 Years of experience as an LPN 2 Years of supervisory experience. As an In-Home Aide and Companion, you’ll have the opportunity to get to know your clients one-on-one, helping them live a healthy, happy and safe life at home. We’re looking for PRN In-Home Aides and Companions (less than 20hrs per week) to float -7a-3p, 3p -11p or 7p – 7a with weekend shifts required. What you'll do: Offer companionship by joining clients on walks or accompanying them on visits Assist and perform tasks delegated by Campus and Home Care nurses to support clients with their ADLs and IADLs Assist Home Care clients with activities such as shopping, transportation to appointments and daily tasks that do not require extensive "hands on" care Support clients with household management tasks, such as caring for their pets, closet organization, light housekeeping, etc. Communicate changes in a resident’s condition to the nurse on duty Maintain chart documentation, and utilize applicable software to do so Always practice safety and infection control methods Maintain confidentiality and privacy of any PHI per HIPAA requirements Participate in ongoing education and training Perform any other duties and/or tasks that may be assigned on an as-needed basis by an authorized supervisor What you'll need: High School or GED equivalent is preferred Must be at least 21 years of age Experience in home care, home health, geriatrics or LTC (Preferred) Ability to travel frequently, with an active driver's license Active CPR certificate or willingness to obtain with six months of joining the team Compensation is $15.00-$16.00 per hour based on experience, plus shift differentials! The Independent Living Nurse Navigator serves as a trusted clinical resource for residents living independently within the Life Plan Community. The Nurse Navigator promotes health, wellness, and successful aging by providing nursing assessment, education, care coordination, resource navigation, advocacy, and transition support. Working collaboratively with Social Work Navigators, chaplains, wellness staff, and healthcare providers, the Nurse Navigator helps residents understand their healthcare needs, navigate complex medical systems, and access appropriate services while supporting their ability to safely remain independent whenever possible. This position focuses on education, coaching, coordination, and early intervention rather than providing ongoing skilled nursing care or replacing What you'll do: Serve as a resource for residents seeking information on healthcare services, insurance, and community resources Provide referrals to specialized healthcare professionals when necessary Coordinate and facilitate support groups for residents dealing with specific health issues like diabetes, heart disease, and mental health challenges Provide one-on-one educational consultations to residents and their families about health-related matters Maintain an up-to-date directory of local health care providers, support services, and other relevant resources Perform any other duties and/or tasks that may be assigned on an as-needed basis by an authorized supervisor What you'll need: Bachelors or Masters prepared nurse Minimum 2 years of care coordination/case management experience Strong organizational skills Experience with geriatric population The compensation for this role is $83,500–$85,000 based on background and experience. In this role you will review the medical needs of potential applicants and determine the appropriate level of care placement. You will coordinate the quality assurance process and infection control program. What you’ll do: Interview, hire, train, and discharge employees in accordance with policies and procedures. Supervise all activities of licensed and non-licensed nursing personnel in the delivery of safe and therapeutic care. Direct care of residents who require minimal to skilled nursing care. Develop, design, and coordinate the implementation of corporate quality assurance processes. Develops, implements, and receives approval for budgets. Communicate resident needs and services to families, other departments, and external representatives. Generate reports as directed by the Director of Health Services in support of daily operations.
