As a Registered Nurse (RN) Supervisor, you will play a pivotal role in our long-term care team, providing exceptional patient care and leadership to our nursing staff. About the Role As a Registered Nurse (RN) Supervisor at Green Meadows Rehabilitation and Nursing Center, you will be responsible for supervising and supporting nursing staff on assigned units, overseeing the implementation of resident care plans, and conducting regular nursing unit rounds. You will also ensure proper documentation of incidents, complaints, and clinical updates, and promote a team-oriented environment focused on quality and compliance. Job Summary: As a Resident Nurse Assessment Coordinator (MDS), you will be responsible for processing related to the assessment of residents in our healthcare facility. This includes assessing patients, providing regular reports to doctors and other medical professionals, billing, and working with the MDS system. Responsibilities: Coordinate and complete all MDS assessments (admission, quarterly, annual, significant change, PPS, OBRA, and discharge) within CMS-mandated timeframes. Schedule and lead interdisciplinary team (IDT) care plan meetings; ensure individualized, resident-centered care plans are developed and updated. Accurately code MDS items to reflect resident acuity, ensuring appropriate PDPM Ensure compliance with all federal and state regulations, including OBRA, PDPM, and Quality Measures (QMs).
