Provide counseling, crisis intervention, and support to residents and their families to address psychosocial concerns and promote coping skills and resilience. Advocate for residents' rights and access to resources, including housing, benefits, and community support services. Collaborate with community partners and organizations to facilitate access to resources and services for residents transitioning to permanent housing. Maintain accurate and up-to-date documentation of care plans, interventions, and resident progress in electronic health records (EHR) systems.
