Registered Nurse (RN) Department: Nursing Reports To: Director of Nursing FLSA Status: Non-Exempt Location: Lincoln Village Healthcare Center, Lincoln, IL PAY: $45.00 per hour ($10,000.00 Sign-On Bonus and $5,000.00 Student Loan Bonus available for Full-Time; We offer mileage reimbursement) Hours: 12- Hour Shifts (6-6) All shifts available. What We Are Looking For: The Registered Nurse (RN) provides direct nursing care to residents and supervises the day-to-day nursing activities performed by Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs). The RN ensures care is delivered in accordance with facility policies, physician orders, resident care plans, and all federal and state regulations. What You Will Do: Resident Care Assess residents’ health status and develop/update individualized care plans. Administer medications and treatments as prescribed, ensuring accuracy and timeliness. Monitor and respond to change in residents’ conditions, including notifying physicians and families as needed. Provide skilled nursing procedures such as wound care, catheter care, IV therapy, and tube feedings per physician orders. Assessment & Documentation Complete and document nursing assessments in accordance with facility standards. Accurately chart medications, treatments, and resident care in the medical record. Participate in the completion of the MDS (Minimum Data Set) and other required assessments within regulatory timelines. Maintain accurate records for compliance with state and federal requirements. Leadership & Supervision Provide supervision, direction, and evaluation of LPNs and CNAs. Conduct shift-to-shift handoff reports and ensure continuity of care. Participate in staff training, orientation, and performance evaluations. Promote teamwork and a positive work environment. Safety & Compliance Follow infection control, safety, and HIPAA guidelines at all times. Respond to emergencies and provide appropriate nursing interventions. Maintain a safe environment for residents, staff, and visitors. Comply with all facility, state, and federal regulations. What You Will Bring: Current, valid Illinois RN license in good standing. Graduate of an accredited nursing program (Associate or Bachelor’s degree). Prior experience in long-term care or geriatric nursing preferred. Strong assessment, clinical, and critical-thinking skills. Excellent communication, leadership, and organizational skills. Physical Requirements: Ability to lift/push/pull up to 50 lbs and assist with resident transfers. Frequent standing, walking, bending, and reaching. Ability to work in a fast-paced environment with potential exposure to infectious diseases. Work Environment: RNs work in a skilled nursing facility with frequent contact with residents, families, and healthcare staff. Certified Nursing Assistant (CNA) Department: Nursing Services Reports To: Director of Nursing / Licensed Nurse (RN/LPN) FLSA Status: Non-Exempt Location: Lincoln Village Healthcare Center, Lincoln, IL PAY: $18.00 per hour, plus up to 6.5% subsidy DOE ($7,500.00 Sign-On Bonus available for Full-Time) HOURS: Full-Time 6:00AM-6:00PM, every other weekend PRN 6:00AM-6:00PM PRN 6:00PM-6:00AM What We Are Looking For: The Certified Nursing Assistant (CNA) provides direct resident care under the supervision of licensed nursing staff. CNAs assist residents with activities of daily living (ADLs), monitor and report changes in condition, and support residents’ comfort, dignity, and safety in accordance with federal, state, and facility standards. What You Will Do: Resident Care Assist residents with ADLs, including bathing, dressing, grooming, toileting, oral care, eating, and mobility. Provide safe resident transfers and ambulation, using proper body mechanics and equipment (gait belts, lifts, etc.). Measure and record vital signs, height, weight, intake, and output as directed. Assist with range-of-motion exercises and positioning to maintain skin integrity and prevent pressure injuries. Observation & Reporting Monitor residents’ physical and emotional status and promptly report changes to the nurse. Document care provided and observations in accordance with facility policies and procedures. Resident Support Encourage resident participation in activities and social interactions. Maintain residents’ privacy and confidentiality at all times (HIPAA compliance). Respond promptly to call lights and resident requests. Safety & Infection Control Follow facility policies for infection prevention, hand hygiene, and use of personal protective equipment (PPE). Keep resident care areas clean, safe, and hazard-free. Participate in emergency procedures and safety drills. What You Will Bring: High school diploma or equivalent preferred. Current, valid Illinois CNA certification in good standing. The LPN is responsible for administering medications, performing treatments, monitoring residents’ health status, and supporting the overall care plan to promote optimal health outcomes. What You Will Do: Resident Care Administer medications and treatments as prescribed, in accordance with facility policies and state regulations.
