PRN LPN

LPN (Licensed Practical Nurse) Job Title : LPN (Licensed Practical Nurse) Location: Memory Care Center in Claremore Department: Nursing Reports to: DON/Charge Nurse Position Type: Hourly Job Summary: As a Licensed Practical Nurse (LPN), you will play a crucial role in delivering high-quality, compassionate care to our residents. Your primary responsibilities will include assessing patient needs, providing direct nursing care, and collaborating with the healthcare team to ensure the best outcomes for our residents. You will work in a supportive and dynamic environment where your skills and dedication contribute significantly to the well-being of our residents. Work Environment: Work is performed in a skilled nursing facility with potential exposure to infectious diseases and hazardous materials. Appropriate personal protective equipment (PPE) is provided. Responsibilities: Patient Care: Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician. Monitoring and Assessment: Monitor residents’ vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing. Care Planning: Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident’s needs. Communication: Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team. Compliance: Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols. Support: Provide emotional support and education to residents and their families, helping them understand care plans and health conditions. Team Collaboration: Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care. Qualifications: Licensure: Valid LPN/LVN license in New York Education: Graduation from an accredited LPN/LVN program. Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred. Certifications: Current CPR/BLS certification required. Additional certifications may be required based on state regulations. Communication: Excellent verbal and written communication skills. Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families. Physical Requirements: Ability to lift and move patients, perform physical assessments, and stand for extended periods. Skills: Ability to perform assessments Administer medications Provide wound care. Excellent communication and interpersonal skills are essential. In this role, you will provide high-quality care and support to residents, ensuring their comfort, safety, and well-being. Work Environment: This position operates in a clinical healthcare environment. Responsibilities: Resident Care: Assist residents with activities of daily living, including bathing, dressing, grooming, and personal hygiene. Mobility Assistance: Help residents with mobility issues, including transfers, positioning, and safe ambulation. Vital Signs Monitoring: Take and record vital signs. Documentation: Accurately document all care provided, changes in residents’ conditions, and any other pertinent information in electronic health records. Communication: Report any changes in residents’ conditions or concerns to the nursing staff or supervisor. Supportive Tasks: Assist with feeding, meal preparation, and providing companionship to enhance residents’ quality of life.
