Duties & Responsibilities: As an RN or LPN Unit Manager, you will: Responsible for the completion of the MDS nursing section. May be responsible to work on unit, do medication passes. Participates in Manager on Duty, a weekend on-call program Participates in weekly plan of care meetings, promotes the interdisciplinary team approach. Keeps ADON/DON informed of neighbor status through verbal and/or written communication. Performs regular clinical rounds of neighbors and nursing staff. Monitor neighbors who have episodic illness. Assists nurses with admissions, transfers and all emergency situations including, but not limited to, technical procedures when necessary. Assists physicians as needed. Performs a total physical assessment of all critical neighbors. Makes sure that all documentation regarding the death is complete. Recognizes unsafe and unclean conditions and implements corrective action to provide neighbors and personnel with a healthy, clean, and safe environment. Ensures timely and proper handling of accident/incident reports and checks to see that documentation is accurate/complete and that proper follow up is performed. Oversees the management of the unit. Reviews charts for completion and documentation accuracy. Ensures correct narcotic count and documentation of controlled substances on unit. Ensures that O2 equipment, enteral feeding pumps, suction machines and other specialized equipment is functioning and that needed supplies are available. Deals effectively, professionally, and tactfully with neighbors and family complaints. Observes undesirable visitors within the facility and assists in maintaining internal and external security. Maintains staff morale and promotes their working together as a unified team. Prepares, reviews, and submits written documentation to the ADON/DON on personnel’s performance and any disciplinary action taken for conduct contrary to facility policy. Responsible for supervising, evaluating, counseling and disciplining personnel on assigned unit. Assists in orientation and on the job training of new employees and ongoing education of staff. Performs direct neighbor care and other job-related duties as required. RESPONSIBILTIES: Admissions Coordination: Serve as the primary point of contact for new admissions/readmissions, ensuring all aspects of the process are handled professionally and efficiently. Coordinate pre-admission assessments/evaluations with the interdisciplinary team to evaluate potential neighbors' medical and nursing care needs. Communicate effectively with the admissions team and the transferring facility (hospital, community or other care setting). Clinical evaluation: Conduct thorough evaluations of incoming neighbors, including physical, mental, and emotional health evaluations. Review medical histories, medications, and provider’s orders to develop initial care plans. Communicate and document any special needs or considerations for each resident. Review and support neighbors and family in any advanced directive care needs Documentation and Compliance: Complete and ensure accuracy of all admission paperwork, including discharge summaries, admission checklists consent forms, medical records, and regulatory documentation. Maintain compliance with state and federal regulations, including Medicare/Medicaid requirements and HIPAA standards. Update the electronic health record (EHR) system with all relevant information promptly. Ensure an active transition of the neighbors entering the facility from another care setting. Resident and Family Support: Welcome neighbors and their families upon admission/re-admission, providing a thorough orientation to the facility and its services. Act as a liaison between residents, families, and the healthcare team, addressing questions and concerns with empathy and professionalism. Provide education to residents and families about care plans, medications, and facility policies. Collaborate with unit-based staff to ensure room readiness Collaboration and Communication: Work closely with the admissions team, nursing staff, and other departments to coordinate the arrival and placement of new/returning neighbors. Collaborate with external healthcare providers, hospitals, and referral sources to ensure continuity of care. Quality Assurance: Monitor and evaluate the admissions process, identifying areas for improvement and implementing solutions. Ensure that neighbors’ initial care plans align with their specific needs and that care delivery meets the facility’s standards. Other Duties: Act in a nursing supervisor capacity when needed. Perform bedside nursing duties such as medication and treatment administration, wound care as well as documentation of care and C.N.A. oversite when needed at the discretion of the director of nursing services, assistant director of nursing services or the nursing supervisor. The admission nurse is expected to participate in the manager on duty rotation as are all nurses in leadership positions. RESPONSIBILITIES: Plans, develops, directs, implements, and monitors orientation and education of all facility personnel. Plans, coordinates, and conducts Certified Nursing Assistant Program. Develops instructional materials to assist new personnel in becoming oriented to facility operational techniques. Schedules, with Human Resources and Administration, facility tours to acquaint new personnel with overall operation and interrelationship of facility services. Determines effectiveness of orientation materials and procedures through practice sessions. Sets up demonstrations of nursing service policies and equipment to acquaint facility staff with new equipment to make them more familiar with established policies and equipment. Plans, coordinates, and conducts regular and special inservice training sessions for facility staff to acquaint them with new procedures and policies and current trends and developments in resident/patient care techniques and employee performance, and to provide opportunity for individual members to develop to their full potential. Keep current on the latest developments by attending professional seminars, institutes, and reading professional journals. Assists Department Heads, Managers, Supervisors and Charge Nurses in planning and implementing staff development programs on their units. Keeps bulletin/informational boards current by listing information on programs, meetings, seminars, etc. and promotes appropriate staff attendance at these meetings. Plans special training sessions for staff members as needed and/or required. Participates in writing and maintaining policies, procedures, and forms. Makes suggestions for changes or clarification in policies and procedures. Completes Education Department reports and maintains Education Department records and documentation (including attendance records) in accordance with Federal and State Regulations and standards for Long Term Care Provides staff with supervision as needed. Works flexible hours as necessary to provide staff development for all staff members. Meets periodically with Department Heads to assess and plan staff education needs. May participate in staff selection process, including interviewing Serves as a member of departmental committees, creating a strong and visible role model for a team approach to problem resolutions and furture planning May fulfill the role of shift supervisor /staffing person if necessary. Develops an educational plan for a NATP program. REQUIREMENTS: NYS Registered Nurse (RN) license require Experience as Charge Nurse, Supervisor or Nurse Educator. Experience/ability to make independent decisions ; ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures, etc., that are necessary for providing quality education and nursing care. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to the health care field.
