Perform additional nursing and clinic duties within the LPN scope of practice as assigned. Qualifications: Current Florida LPN license in good standing. Graduate of an approved practical nursing program. Current BLS certification. Previous medical clinic, primary care, chronic care management, or care coordination experience preferred. Strong communication, organization, and documentation skills. Comfortable communicating with patients by telephone and working with electronic medical records. Ability to work independently while collaborating closely with providers and clinic staff. Knowledge of common chronic conditions, including diabetes, hypertension, COPD, and heart disease, preferred.
