The Position: We're looking for passionate Home Health Aides provides personal care and assigned therapy exercises within the patient's home; assists patient in reaching the maximum level of self care. Provide activities of daily living (ADL) care including bathing, hair care, nail and skin care, and oral hygiene. Serve as an integral member of the interdisciplinary team. Recognize and report changes in skin condition to Case Managers. Requirements: High School Diploma or equivalent. / Minimum age of 18. Completion of state-approved Nurse Aide Training Program required. Certified Nurse Aide (CNA) certification with the State of Hawaii preferred. Must maintain current BLS Certification. The Position: We're looking for passionate Registered Nurses to help us with providing professional nursing care to residents following established policies and procedures of the facility. Administers medications/treatments as ordered by the physician and according to policy and procedures. Takes telephone orders from physicians and transcribes orders to new medication administration record, the pre-printed physician order sheet, resident chart, treatment/care plans as appropriate. Maintains & orders appropriate inventory of resident medications and medical stock supplies. Conducts accurate physical assessments of residents, determines their needs and implements appropriate interventions to meet those needs. Performs assessments on admission, transfer and/or discharges. Requirements: Graduate of an accredited school of nursing. Current license as a Registered Nurse in the State of Hawaii required. Plans, develops and organizes work tasks; coordinates supervise and evaluates employees. Coordinates the unit's services and activities in consultation with the Director of Nursing. Plans and supervises resident care. Continually assesses residents' physical conditions and participates in resident planning conferences.
