Job Title: State Tested Nursing Assistant (STNA/CNA) Location: The Pavilion at Piketon Job Type: Full-Time, Part-Time Salary: Negotiable About Us The Pavilion at Piketon is a skilled nursing and rehabilitation center located in Piketon, OH. At The Pavilion at Piketon, our mission is simple: “To make a positive difference in the lives of those we serve by providing competent, compassionate and outcome-oriented care – every day.” Position Summary The State Tested Nursing Assistant (STNA/CNA) provides direct care to residents under the supervision of licensed nursing staff. This includes assisting with daily living activities, ensuring resident safety and comfort, creating better quality of life to our residents. Key Responsibilities Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned. Assists resident with or performs activities of daily living for resident in accordance with care plans and established policies and procedures. Assists resident with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. Coordinates dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake. Delivers nutritional supplements to residents at assigned times and provides assistance as necessary to ensure intake. Documents intake accordingly. Assists with weighing residents according to facility policy, and records weight in designated locations. Reports weight changes to nurse and supervisor. Assists with tracking the condition of the resident’s skin. Reports any presence of pressure areas, skin breakdown or skin tears to nurse and supervisor. Assists nursing staff in carrying out toileting program activities. Completes flow sheets daily to indicate that the specified task was done. Documents the progress of each resident as directed by supervisor. Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e. resident refusal, reassigned to nursing unit). Reports changes in a resident’s condition to the resident’s nurse and supervisor. Other duties as assigned. Qualifications State Tested Nursing Assistant license in good standing with the state. High school diploma or equivalent. Current CPR/BLS certification. Licensed Practical Nurse (LPN) Night Shift

Job Title: Licensed Practical Nurse (LPN) Location: The Pavilion at Piketon Job Type: Full-Time, Part-Time Salary: Negotiable About Us The Pavilion at Piketon is a skilled nursing and rehabilitation center located in Piketon, OH. Key Responsibilities Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures. Follows the nursing model to promote holistic care for the residents. Ensures that policies and procedures are complied with by nursing personnel assigned. Participates in the admission, discharge and transfers of residents as required. Assesses for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly. Transcribes physician orders to medical record and carries out orders as written. Prepares and administers medications as per physicians’ orders and observes for adverse effects as indicated. Performs wound treatments as per physicians’ orders, observes for changes and documents accordingly as indicated. Schedules follow up appointments for residents and transportation needs as indicated. Participates in management team meetings as indicated to discuss census changes, resident changes in status or care need, or complaints or concerns. Other duties as assigned. Unit Manager/Wound Nurse (RN) – Long-Term Care / Skilled Nursing Location: The Pavilion at Piketon Job Type: Full-time Position Summary We are seeking an experienced and motivated Unit Manager (RN/LPN) to oversee clinical care and lead nursing staff within our long-term care/skilled nursing facility. This role is responsible for staff supervision, care coordination, clinical oversight, and resident outcomes , while supporting a high standard of care and regulatory compliance. The ideal candidate brings strong leadership, clinical expertise, and a passion for geriatric care , along with the ability to guide and develop nursing staff in a fast-paced environment.
