Provides certified nursing assistant services to assigned residents in accordance with care plans, facility policies and procedures and at the direction of supervisor(s). Required Qualifications Minimum requirements include the following: Certified Nursing Assistant in good standing with the state of South Carolina. High school diploma or equivalent. Must have no disciplinary action in effect against certification. Major Duties and Responsibilities Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned. Assists resident with or performs activities of daily living for resident in accordance with care plans/personal care records (PCR’s) and established policies and procedures. Assists resident with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. Coordinates dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake. Delivers nutritional supplements to residents at assigned times and provides assistance as necessary to ensure intake. Documents intake accordingly. Assists with weighing residents according to facility policy, and records weight in designated locations. Reports weight changes to nurse and supervisor. Assists with tracking the condition of the resident’s skin. Reports any presence of pressure areas, skin breakdown or skin tears to nurse and supervisor. Assists nursing staff in carrying out toileting program activities. Completes flow sheets daily to indicate that the specified task was done. Documents the progress of each resident as directed by supervisor. Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e. resident refusal, reassigned to nursing unit). Reports changes in a resident’s condition to the resident’s nurse and supervisor.

Provides direct nursing care to the residents and supervises the day-to-day nursing activities performed by the certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures. Required Qualifications A Nursing Degree from an accredited college or university or a graduate of an approved RN program. Prior experience in long-term care preferred but not required. Current unrestricted license as a Registered Nurse (RN) in the state of South Carolina. Current CPR certification. Major Duties and Responsibilities Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures. Ensures that policies and procedures are complied with by nursing personnel assigned. Participates in the admission, discharge and transfers of residents as required. Observes for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly. Initial Health Assessment Staff Nurse - Ready for Surgery

Full Time, 1st Shift, 8:00-4:30pm, Monday-Friday **This position will be in the Ready for Surgery Department at Tower Point** The Preoperative Phone Screening RN is responsible for conducting telephone-based preoperative assessments to ensure patients are prepared for their scheduled surgical procedures. This role involves reviewing medical histories, identifying potential risks, providing preoperative instructions, and communicating critical findings to the Ready for Surgery team. The LPN works closely with registered nurses (RNs) and other healthcare providers to facilitate a smooth and safe surgical experience for patients. Participates in any other job related duties directed by the Practice Manager or Director. Special Qualifications Registered Nurse (RN) license in good standing.
