LPN/RN Admissions Nurse

$45 - $59 / hour
We’re looking for an experienced LPN or RN to join our Admissions team at our rehab center. This role is a good fit for someone who understands long-term care, is confident in their clinical judgment, and can move quickly while staying organized. You’ll be responsible for reviewing incoming patients, assessing their needs, and helping make sure each admission is appropriate and set up for success. You’ll work closely with hospitals, providers, and our internal team to keep the process smooth and safe. Job Description Job responsibilities: Daily rounds to monitor resident condition and staff performance Coach and/or counsel staff providing care on your unit Review of documentation to ensure compliance and accurate charting Assist floor staff with emergencies, changes of condition and MD calls as needed Participates in Care Conferences and quarterly reviews. Ensures weights are obtained per policy, reviewed and referrals made as appropriate Participate in wound rounds weekly and assist with documentation as needed Participate in psychotropic review monthly with follow up on team recommendations Track and ensure consultant recommendations are completed and results documented Assist in the admission and discharge process for residents Review/investigate Incident reports utilizing root cause analysis (the 5 why's) to identify cause and implement appropriate preventative measures On-call rotation (approx every 5 weeks) Performs duties as a floor nurse if staffing crisis occurs. Staff Development Coordinator (RN or LVN)

$100,000 - $108,000 / year
Required Qualifications v Minimum requirements include the following: · A nursing degree from an accredited college or university, or be a graduate of an approved LPN/RN program. · Two years of experience as an LPN/RN. · Eligible to participate in federally funded health care programs. v Must also meet state requirements for relevant licensures or certifications and have no disciplinary action in effect against professional license. v Extensive knowledge of current standards of practice and the rules, regulations, and laws related to the long-term care industry. Ability to teach this knowledge to others. Major Duties and Responsibilities Participates in the completion of the facility assessment at least annually for determining the knowledge and skills required among staff to meet residents’ needs. Identifies and prioritize the facility’s educational needs through the completion of an educational needs assessment, based on the facility assessment. Works closely with the facility’s Compliance Officer and other leaders in developing a training plan that addresses all regulatory requirements and any other topics identified in the facility assessment and/or needs assessment. Develop educational calendars in accordance with the plan. Plans, schedules, conducts, and oversees orientation programs for all personnel. Develops, schedules, and directs refresher training, as necessary, for all personnel. Tracks attendance/completion of training to ensure that all required training is received by all personnel in a timely manner. Reviews educational content to ensure the information reflects current standards of practice and facility policies and procedures. Develops or revises content as needed. Utilizes outside resources for content development, such as consultants, content experts, or electronic learning systems. Collaborates with department managers for developing competencies required for meeting residents’ needs. Assists with evaluating competencies, depending on the knowledge-based, technical, or behavioral nature of the competency. Cooperates with the Administrator during routine evaluation of the training program’s effectiveness. Schedules trainings with flexibility to ensure limited disruption of resident care, and ample opportunity for staff to attend. Posts notices of trainings in advance. Maintains all training records, reports, instructional materials, and speaker qualifications related to each educational program. Provides opportunities for staff to obtain continuing education credits for maintaining certifications/licensure. Maintains all documentation related to continuing education. Protects residents from abuse, and cooperates with all investigations. v Reports any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy. v Participates in all life safety and emergency drills and trainings. Fulfills responsibilities as assigned during implementation or activation of the facility’s emergency plan. v Reports work-related injuries and illnesses immediately to supervisor. v Follows established infection control policies and procedures. v As a condition of employment, completes all assigned training and skills competency. v Remains current on new developments by attending professional institutions, reading professional journals, attending professional seminars, or reading new laws, rules, and regulations. v Assists with admission, discharge, or care of residents as needed. v Uses proper lifting and body mechanics while delivering care to residents. v Performs administrative duties as assigned. v Serves on, participates in, and attends facility, departmental, or committee meetings as required or appointed. v Assists with developing and adhering to an education budget. Restorative Nursing Assistant

Job description Assists with active and passive therapeutic exercises under direct supervision of Director of Nursing Services (DNS) or designated Nurse Manager. Instructs, motivates, and assists residents to learn and improve functional activities, such as eating, transfer, ambulation, and daily-living activities, and completes documentations. Observes residents during treatments and compiles and evaluates data on residents’ responses to treatments and progress and reports orally or in writing to Nurse Manager. Confers with members of restorative staff and other health team members, individually and in conference, to exchange, discuss, and evaluate resident information and participation for planning, modifying, and coordinating treatment programs. Records resident information on the restorative flow sheet, according to policy. Assists with resident meal service as assigned. Answers signal lights, bells, or intercom and provide resident care and services needed. Transports resident to and from restorative program. Meets with the Nurse Manager a minimum of monthly to review each resident’s performance in their restorative program. Knowledge, Skills, And Abilities: Previous long-term care experience. Completed on-the-job training by a qualified therapist (physical or occupational). Ability and willingness to work well with residents and staff Education, Licensure, And Experience: High school diploma or equivalent.
