Job Description The Adult Day Care Center located at Waters Place, Bronx is currently seeking a part time Registered Nurse ( RN) to join our team! The work schedule is 30 hours per week, Monday to Friday from 9 am to 3:30 pm, with an occasional Saturday. The part time RN is entitled to receive a sign-on bonus of $2500. Job Summary: Responsible for providing and coordinating multidisciplinary health services for older adults in the Day Care Center . Works with the physician and other team members in implementation of plan of care. Duties And Responsibilities Conducts comprehensive clinical assessments identifying the service needs of each registrant upon intake and thereafter on a quarterly basis Ensures that for each registrant the findings of the Nursing evaluations, the Nursing Care plan and the recommendations for Nursing follow-up are documented in the clinical record Ensures that MD orders are completed in accordance within the specified time frame; follows up regularly to ensure receipt of MD orders Provides medication administration, prefills, and other skilled Nursing services in accordance with MD orders Assigns CAN tasks in accordance with the Care Plan and oversees the daily completion of the tasks Provides and coordinates cost-efficient patient care that is under the patients’ insurance plan and approved by a physician. Uses diagnosis coding skills to accurately rate the acuity of each registrant Evaluates patient diagnosis and works with Clinical Supervisor, Patient Services, Medical Director, and physician to ensure clinical services and Nursing interventions meet the goals of the comprehensive Care plan Participates meetings to report adherence to frequency and duration of services as per physicians’ order. Reports patients’ progress to team. Communicates frequently with the clinical team to ensure adherence to services at the frequency and duration ordered by the physician and outlined in the comprehensive Care Plan. Establishes a plan of care with input from appropriate multidisciplinary health team members based on goals mutually acceptable to the patient and family. Documents content and process of all services provided. Monitors the effect of the care plans on the patient health status and informs physician and other personnel of changes in the clients’ needs. Appraises care provided, revises goals and interventions to patients’ care as needed. Educates the older adults and family regarding the disease process, self-care techniques and prevention strategies. Initiates and maintains written and verbal communications to assure optimal patient care. Performs other relevant duties as needed. Requirements Skills & Requirements License and current registration to practice as a Registered Professional Nurse in New York State. Graduate of an accredited school of nursing, BSN preferred. A minimum of one year experience in acute and subacute care and or minimum of one year experience in a Adult Care Center preferred. Bilingual ( Spanish /English) and UAS certified preferred.

Job Description The Manhattan Division of The New Jewish Home is searching for a full time in person Admissions Evaluator to join their team. Onsite location is at 120 West 106th Street, NY, NY 10025. The Duties And Responsibility Of The Admissions Evaluator Are Reviews PRI’s and all supporting clinical documentation for appropriateness to the facility. Serves as a liaison to hospitals through the metropolitan area and telephone consultations to confer with hospital staff and to seek out and evaluate potential applicants for admission to the facility. Completes all required clinical pre-admission assessment forms on all potential evaluated, gathering additional medical and social information as need for a thorough assessment. Begins the application process via telephone by disseminating information about The New Jewish Home programs; may gather psychosocial and financial information to complete applications with applicants and/or applicants. Documents all applicant clinical and psycho-social assessments. Submits monthly statistics and reports as requested by the Director of Admissions. Contacts Managed Care Companies to obtain pre-authorization; reviews level requirements to obtain appropriate level prior to admissions. Conducts tours of the facility with potential patients, families and or representatives. Interacts with HMO Case Managers prior to admissions, to review skilled needs and maximization of level. Weekend and Holiday admission coverage as needed. Performs other duties as required. Skills & Requirements JOB QUALIFICATIONS: Registered Nurse in the State of New York. MDS experience and PRI certification would be a plus. Previous experience in post acute admissions processes Acute Care experience preferred. Bilingual is a plus. Specialized Skills And Competencies Responds politely and helpfully to telephone and in-person requests for service consultations. Excellent writing and clinical assessment skills. Good working relationships with staff and referral services. Ability to multi-task and work accurately in a fast paced environment. Knowledge of Electronic Medical Record, Microsoft outlook, Word, and Excel. Required to speak and write in an understandable manner. Bilingual (English/Spanish) a plus.
