Job Description The Nursing Department on the Manhattan campus is seeking a full time Nurse Manager to join our team. Work schedule is day shift on weekdays , with availability to work on weekends every 4th week of the month. This position is entitled to an Incentive bonus of $ 5000, which is distributed as follows: End of 6 months of initial employment - $ 2500 End of 1 year employment- $ 2500 The Nurse Manager for Sub-Acute floors is responsible for assuring approved quality standards of care and compliance with established policies and procedures in the provision of resident/client care. Duties And Responsibilities Directs and supervises the delivery of nursing care within an assigned unit. Delegates the responsibility for direct resident care to the appropriate nursing staff member. Monitors care to assure care is provided as directed by the resident plan of care. Effectively communicates resident and unit needs to Nursing Administration. Utilizes a process of data collection, assessment and care planning to meet the needs of resident.Facilitates communication and problem solving with other disciplines to develop a plan of care which will address these goals. Responsible for assuring the Resident Care Plan is revised as necessary to reflect the current needs of the resident. Participates with Nursing Administration in meeting clinical and budget goals. Evaluates staff assigned to her/his assigned unit to determine staff competencies and/or the need for education. Assists, as appropriate in the education of staff. Participates in the counseling and discipline of staff. Maintains personal competencies to assure the ability to provide resident care,evaluate the competencies of staff assigned to his/her unit and to serve as a role model for unit staff. Communicates resident needs, strengths and conditions to appropriate staff members through unit conferences and in written format. Requirements Skills & Requirements Valid Registered Nurse license in New York State. Bachelors in Nursing is strongly preferred. A minimum of four years experience as a Nursing Manager/Supervisor and/or Charge Nurse in long term care setting/sub-acute settings is required . Demonstrated nursing and management leadership skills are required. Job Description The Adult Day Care Center located at Waters Place, Bronx is currently seeking a part time Registered Nurse ( RN) to join our team! The work schedule is 30 hours per week, Monday to Friday from 9 am to 3:30 pm, with an occasional Saturday. The part time RN is entitled to receive a sign-on bonus of $2500. Job Summary: Responsible for providing and coordinating multidisciplinary health services for older adults in the Day Care Center . Works with the physician and other team members in implementation of plan of care. Duties And Responsibilities Conducts comprehensive clinical assessments identifying the service needs of each registrant upon intake and thereafter on a quarterly basis Ensures that for each registrant the findings of the Nursing evaluations, the Nursing Care plan and the recommendations for Nursing follow-up are documented in the clinical record Ensures that MD orders are completed in accordance within the specified time frame; follows up regularly to ensure receipt of MD orders Provides medication administration, prefills, and other skilled Nursing services in accordance with MD orders Assigns CAN tasks in accordance with the Care Plan and oversees the daily completion of the tasks Provides and coordinates cost-efficient patient care that is under the patients’ insurance plan and approved by a physician. Uses diagnosis coding skills to accurately rate the acuity of each registrant Evaluates patient diagnosis and works with Clinical Supervisor, Patient Services, Medical Director, and physician to ensure clinical services and Nursing interventions meet the goals of the comprehensive Care plan Participates meetings to report adherence to frequency and duration of services as per physicians’ order. Reports patients’ progress to team. Communicates frequently with the clinical team to ensure adherence to services at the frequency and duration ordered by the physician and outlined in the comprehensive Care Plan. Establishes a plan of care with input from appropriate multidisciplinary health team members based on goals mutually acceptable to the patient and family. Documents content and process of all services provided. Monitors the effect of the care plans on the patient health status and informs physician and other personnel of changes in the clients’ needs. Appraises care provided, revises goals and interventions to patients’ care as needed. Educates the older adults and family regarding the disease process, self-care techniques and prevention strategies. Initiates and maintains written and verbal communications to assure optimal patient care. Performs other relevant duties as needed. Requirements Skills & Requirements License and current registration to practice as a Registered Professional Nurse in New York State. Graduate of an accredited school of nursing, BSN preferred. A minimum of one year experience in acute and subacute care and or minimum of one year experience in a Adult Care Center preferred. Bilingual ( Spanish /English) and UAS certified preferred.
