Job Title: Licensed Practical Nurse (LPN) Location: The Pavilion at Jacksonville Job Type: Full-Time, Part-Time Salary: Negotiable About Us The Pavilion at Jacksonville is a skilled nursing and rehabilitation center located in Jacksonville, FL. At The Pavilion at Jacksonville, our mission is simple: “To make a positive difference in the lives of those we serve by providing competent, compassionate and outcome-oriented care – every day.” Position Summary The Licensed Practical Nurse (LPN) may provide direct nursing care to the residents and supervises the day-to-day nursing activities performed by certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures. Key Responsibilities Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures. Follows the nursing model to promote holistic care for the residents. Ensures that policies and procedures are complied with by nursing personnel assigned. Participates in the admission, discharge and transfers of residents as required. Assesses for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly. Transcribes physician orders to medical record and carries out orders as written. Prepares and administers medications as per physicians’ orders and observes for adverse effects as indicated. Performs wound treatments as per physicians’ orders, observes for changes and documents accordingly as indicated. Schedules follow up appointments for residents and transportation needs as indicated. Provides nursing leadership to nursing personnel assigned to the unit. Other duties as assigned. Qualifications Prior experience in long-term care preferred but not required. A Nursing degree from an accredited college or university. Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state. Current CPR certification. Job Title: Assistant Director of Nursing (ADON) Location: The Pavilion at Jacksonville Job Type: Full-Time Salary: Negotiable Position Summary The Assistant Director of Nursing (ADON) supports the Director of Nursing in overseeing the clinical operations of the nursing department to ensure the highest quality of care is provided to residents. The ADON plays a vital leadership role in promoting a culture of safety, excellence, and continuous improvement in resident care. Key Responsibilities Assists the Director of Nursing in planning, developing, organizing, implementing, evaluating and directing the day-to-day operations of the Nursing Services department, as well as its programs and activities, in accordance with current state and federal laws and regulations. Interprets and communicates policies and procedures to nursing staff, and monitors staff practices and implementation. Participates in QAPI or facility assessment activities as needed, such as carrying out duties assigned as part of a performance improvement committee. Facilitates, serves, attends or participates in various committees of the facility as appointed. Performs rounds to observe residents and ensure nursing needs are being met. Conducts observations of nursing care and supervises development of in-service education to ensure nursing staff is competent in current knowledge and skills. Promote teamwork, mutual respect, and effective communication. Other duties as assigned. Qualifications A Nursing Degree from an accredited college or university. Must have, as a minimum, three (3) years of experience as a supervisor in a hospital, nursing home, or other related health care facilities. Current unrestricted license as a Registered Nurse (RN) in practicing state. Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines pertaining to long-term care. Current CPR certification.
