Nurse Practitioner Preoperative Anesthesia

Job Description Under the general supervision of the Preoperative Anesthesia Clinic Medical Director, Pain Clinic Medical Director, and Chief of Anesthesia, and in accordance with standardized procedures, the Nurse Practitioner (NP) role will work clinically in the Preoperative Anesthesia Clinic (PAC). The Preoperative Anesthesia Clinic is an ambulatory clinic responsible for the preoperative assessment and optimization of patients undergoing surgery. The clinic offers telemedicine and in-person consultations. The NP role provides clinical and operational support to the Preoperative Anesthesia Clinic at Zuckerberg San Francisco General Hospital. General responsibilities are obtaining a health history, performing a physical assessment, identifying problems and formulating a plan that includes further diagnostic evaluation, risk stratification and counseling. Qualifications Required Qualifications: Graduation from a California Board of Registered Nursing approved or verified school of nursing with a master’s degree in nursing or a Doctor of Nursing Practice (DNP). Nurse Practitioner 1 (less than six (6) months of experience as an NP): At least one year of recent work in outpatient neurology and/or neurosurgery clinic Nurse Practitioner 2 (at least six (6) months of NP experience): At least two years of recent work in outpatient internal medicine or critical care. A minimum one (1) year of recent clinical experience as a Nurse Practitioner. Registered Nurse Quality Improvement

Job Description Based in Sacramento. Travel is required; 2 to 3 days a month to sites in California. The California Department of Public Health (CDPH), Office of AIDS (OA), Registered Nurse Quality Improvement provides specialized nurse consultation, technical assistance, training, and education to CDPH/OA staff and Medi-Cal Waiver Program (MCWP) personnel. This role ensures MCWP participants receive appropriate care, agencies meet clinical compliance requirements, and CDPH/OA fulfills federal waiver mandates. Responsibilities include evaluating Nursing Facility Level of Care, conducting Program Compliance Reviews, and documenting progress on CMS performance measures. The nurse facilitates trainings, develops clinical plans and reports, analyzes medical data, identifies barriers, and provides recommendations. Heavy travel is required. Medical Assistant - Orthopedics

Job Description Employment Duration: 3 months Under the supervision of the UCSF Health Nurse Manager and guided by the policies and procedures of Ambulatory Services, the Hospital Assistant III assists in the delivery of care to patients at UCSF Health Stanyan and Hyde Ambulatory Clinics. The Hospital Assistant III is primarily responsible for members of their primary care or multi-specialty teamlet and provides back-up support to other teamlets. This role plays a critical part in the patient experience by optimizing clinic flow, conducting patient outreach, and working collaboratively with the team to create a patient-centered environment. This position floats between the front and back office, performing both clerical and clinical duties. Work includes a variety of personal care and nursing care procedures to support positive care outcomes. The Hospital Assistant III performs assigned patient care and laboratory procedures in support of services provided within the practices and is required to assist in all practice functions, including front and back office operations. The Hospital Assistant III consistently projects a professional and courteous demeanor, maintains high service standards, and complies with Medical Center House and Telephone Standards. FM Inpatient Navigator

Job Description The Family Medicine Inpatient Navigator will support the clinical and logistical needs of patients admitted to the Family Medicine Inpatient and Parent Baby Services as they transition from the hospital to home and other community settings. Despite a long history of caring for vulnerable and marginalized patients with complex chronic illness, as well as extensive efforts by our physicians, pharmacists, nurses, social work and nursing partners, we continue to have difficulty safely bridging patients to the community for continued care and mitigating unnecessary readmissions. Our aggregate 30-day readmission rate has at times been as high as 20-25%, a level well above Centers for Medicare & Medicaid Services (CMS) targets. This position helps to monitor this rate and improve it by focusing resources and support on collaborative planning and communication between patient, inpatient and outpatient clinical teams and community-based organizations. The FM Inpatient Navigator will serve patients and their designees to ensure safe discharge and re-integration into the community during and after discharge from the hospital. The navigator will be an integral part of the care team, attending daily multidisciplinary rounds and working closely with physicians, nurse practitioners, nurses, pharmacists and social workers.
