Nurse Practitioner or Physician Assistant - Cardiothoracic Surgery/OR (UMMC)

University of Minnesota Physicians seeks a Physician Assistant or Nurse Practitioner who will provide care and assist in surgery for patients on the Cardiothoracic Surgery Team at M Health Fairview University of Minnesota Medical Center. In this Role you will: Cover inpatient floor; endoscopic vessel harvesting and OR first assisting. Provide care to patients with cardiac conditions, ranging from the routine to the complex (such as end-stage heart failure and transplant). Our specialists provide comprehensive surgical expertise. Valve repair for aortic and mitral valve pathology is commonly performed, employing innovative techniques of leaflet excision, chordal replacement, annuloplasty, and valve resuspension. In addition to harvesting saphenous vein for coronary artery bypasses. Coordinate and provide surgical care for adults with selected cardiothoracic diseases throughout the course of evaluation and treatment, including hospital admission, surgical intervention, inpatient care, consultations, and discharge planning. This includes clinic follow-up, providing information to referring physicians and institutions, and patient /family education. Provide and coordinate continuity of care between the hospital floor setting, operating room, and clinic. This individual must show commitment to exemplary patient care. Share responsibility for assessment and management of patient in cardiothoracic surgical services. Write admission and daily notes, order and direct discharge planning and follow up. Work to improve the quality of care and to decrease length of stay on the inpatient service. Obtain patient histories, perform physical and psychological health assessments, provide patient and family education and counseling, ensure continuity of health care services. Provide direct management of postoperative care in the clinical settings of the ICU, step-down units, and wards including removing chest tubes, drains and temporary pacemaker wires. University of Minnesota Physicians is seeking a Certified Nurse Practitioner or Certified Physician Assistant to join our Cardiovascular Genetics practice at the M Health Fairview Clinics and Surgery Center in Minneapolis. In This Role, You Will: Work in the ambulatory space to p rovide comprehensive, longitudinal ambulatory care for adults with inherited or suspected genetic cardiovascular conditions, including cardiomyopathies, inherited arrhythmia syndromes, aortopathies, and other heritable cardiovascular diseases. Perform new and follow-up patient evaluations, including comprehensive histories, physical assessments, review of diagnostic testing, and development of individualized plans of care in collaboration with the Cardiovascular Genetics team. Provide ongoing management and surveillance for patients with inherited cardiovascular disease, including coordination of appropriate cardiac imaging, rhythm monitoring, laboratory testing, and other diagnostic evaluations. Work closely with cardiovascular genetic counselors and physician specialists in the evaluation of patients and families with known or suspected inherited cardiovascular disease. Participate in the review and interpretation of genetic testing results within the APP's scope of practice and in collaboration with genetic counselors and physicians, incorporating results into individualized cardiovascular management, surveillance plans, and the importance of appropriate screening for at-risk family members. Review and incorporate multigenerational family histories and pedigrees into cardiovascular risk assessment and clinical decision-making. Help identify patients who may benefit from genetic evaluation and facilitate appropriate referrals for genetic counseling and testing. Collaborate closely with cardiovascular genetics, genetic counseling, electrophysiology, heart failure, cardiovascular imaging, vascular/aortic specialists, and other multidisciplinary partners to provide coordinated, patient-centered care. Participate and help lead monthly Cardiovascular Genetics Multidisciplinary team meetings. Communicate recommendations and plans of care with referring physicians, primary care providers, and outside institutions to promote continuity of care. Provide General Cardiology care, including evaluation and longitudinal management of cardiovascular conditions with an emphasis on cardiac sarcoidosis. Support timely patient access through independent APP clinics and collaborative physician/APP practice. Participate in the continued development and growth of the Cardiovascular Genetics program, including opportunities to help develop clinical workflows, care pathways, patient education, and best practices for inherited cardiovascular disease. Demonstrate a commitment to exemplary patient care, evidence-based practice, and appropriate utilization of healthcare resources. In This Role, You Will: Be responsible for inpatient only duties at the University of Minnesota Medical Center East and Riverside campuses in collaboration with an academic service including attending physicians, residents and students.
