Home Health Aide/Personal Care Aide

The Pavilion at vestal is looking for Home Health Aides or Personal Care Aides Please see below $19.05/HR POSITION DESCRIPTION Home Health Aide (HHA) Qualifications: 1. HHA Must have a current and valid HHA Certificate 2. HHA Must be at least 18 years of age 3. HHA Must be able to read, understand, and follow resident care plans Reports to: Administrator, Administrator-In-Charge, Supervisor-In-Charge and/or RN Home Health Aide (HHA) Responsibilities: Administers medications and treatments per physician order and in accordance with facility policies and procedures and completes associated documentation. Provide assistance to residents with activities of daily living and personal care as needed according to care plan. Assure that residents are dressed appropriately when in living areas of the home or outside the building. Assist and teach the residents skills in self-help in activities and daily living to maximize their independence and freedom of movement. Measures and records vital signs as ordered by provider or as directed by nurse. Assist a self-directing resident in performing maintenance exercise programs Assist a self-directing resident with use of oxygen equipment Assist a self-directing resident with the changing an ostomy or urinary diversion appliance or dressing when the ostomy is mature and stable Perform administrative requirements, such as completing necessary forms, charts, reports, etc., and submit these as may be required. Report any complaints or grievances made by residents to the Supervisor or Administrator. Participate and cooperate with any facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing Services. Provide personal assistance as needed, requested, or required. Respond to call bells or other signaling calls. Assist the Activities Coordinator in carrying out planned activities. Provide assistance to residents to keep clothes in proper order. Assist residents with making beds as needed. Make beds when residents not able to do so. Provide residents when a clean towel and washcloth as needed. Provide assistance with using, emptying, and cleaning resident equipment (urinals, bedside commodes, bed pans, wash basins, showers, chairs, etc.) as needed. Assure residents receive their meals and snacks and assist them at mealtime as needed. Help orient new residents admitted to the program. Notify Administrator when there is a need for medical attention of any resident or any other change in resident status. Report any accident or incident to Administrator or Supervisor. Help maintain the self-respect, personal dignity, and physical safety of each resident. Work cooperatively with all other staff and relate favorably to residents, their families, and visitors. Be familiar with confidentiality of resident information and adhere to requirements. Be familiar with and be able to apply all of the Assisted living home's accident, fire safety, OSHA, and emergency procedures. Be familiar with the Assisted Living Residents' Bill of Rights and assist residents in exercising their rights. Assure careful treatment of adult care home equipment and utilization of supplies. Read and adhere to all assisted living regulations and policies and employee policies. Participate in any job-related continuing education as required by Administrator. Maintain accurate and detailed documentation as required in resident services, such as progress notes, incidents, and changes in condition, appearance, and behavior of residents. Perform other duties as assigned within scope of practice and in accordance with licensing rules and the home's administrative policies and procedures. The Pavilion at Vestal Park is seeking Personal Care Aides that are interested in being employed at Vestal Park Assisted Living . Candidates must already be certified Personal Care Aides in NYS. Graduates' employment post training will be full time to part time. Graduates must plan to continue employment at Vestal Park for 6 months posttest.
